What Causes Difficulty In Sleeping And Night Mares While On Mirtazapine And Olimelt?

Question: Hi doc, am on the following medication:
Mirtaz 18.75 (15+1/2of7.5) in the night
Olimelt 1.25 in the night
Etilam 0.25. + betacap tr 20 in the morning
I don't get proper sleep, sleep is full of dreams and wake up very early at about 5ish and then find it difficult to go to sleep
Earlier suffered from constipation ( that time was on mirtaz 22.5 + quetiapen 12.5 mor/eve along with the above medication), stomach cramping, pain and burning sensation, though it has improved but still bothers me. What would be your advice?

In some individuals Mirtazapine is known to result in night mares because of increase in REM sleep but you are taking the drug since long so I can't say this with confidence. Reducing the dose of Mirtazapine would help in reducing the nightmares and will improve the sleep. Mirtazapine is a particular drug that in low doses 7.5-15 mg helps to improve the sleep but in high doses has not much effects. So you can wait for 3-4 days to see if the sleep improves and if the symptoms persist then reduce the dose of Mirtazapine to 15 mg per day. This would improve the sleep and you will definitely feel better.

My doc cut down etilam from 0.5 in the morning to 0.25 a week ago and today asked me to stop it, she has asked me to stop 0.25, do you think it can be withdrawal symptoms of etilam? She has decided to increase olimelt to 2.5 from 1.25, do you think it will help. And if my symptoms are coz of withdrawal of etilam, do you think it will settle down and how long will it take?

Hello again

Etilaam is a relatively safe benzodiazepine and doesn't usually result in withdrawal so I don't think reducing the dose from 0.5 to 0.25 resulted in these symptoms. You can keep the drug on SOS basis for anxiety there is no need to worry.

Olanzapine can be increased to 2.5 mg and this should improve the anxiety. But wait for few days and remain on same doses, changing the doses at frequent intervals would not help much.

Dear doc, what along with mirtaz and olimelt, I can take for my IBS? It is irritaring, though it has slightly improved but still bothers me. Someone made a mention of serenace 0.25 thrice a day, dono whether I should take it along with mirtaz and oleanz. Then someone said prothiaden or cloranil, you think these meds would help in my anxiety and IBS? A doctor friend of mine said colospa, don't whether it's good for IBS with constipation? Also, what is tour view on probiotics? Are they good for IBS with constipation? Is it advisable to librium or librax? My doc says its better to stay away benzos. Or I can try just clidinium bromide and will it help?
If you can also help with the name of a good psychiatrist in Mumbai!

- I don't think there is need to take other medicines apart from the medicines you are taking. As your IBS symptoms are relatively better so no need to start one more drug. Sarenace is an antipsychotic and th drug would not help much. In my opinion you should not take this drug.

- Prothiaden or Clonil wouldn't help much. These have mechanism of action almost same as serotonergic drugs. Adding these drugs would not add benefit.

- Yes, probiotics is a good idea. You can start probiotics without any worries.

- Benzodiazepines are risky drugs but if used on SOS basis these drugs are wonder drugs. You can take Libratax on SOS basis.

So just don't think of taking more drugs. These drugs will result in improvement with time.
