What Causes Difficulty In Urination And Scanty Urine Flow?

Question: Hi. In the last week I have been waking up at night to urinate but the amount is small. When i urinate in the date it feels like it is slow, i get a feeling of pressure in the urethra but the flow is slow. This evening i had a strong urge to urinate but when I did, the flow slowed almost to a stop while the urge was still strong.
I had a check up including prostate less than two months ago and my prostate was normal. i was also given all clear of any STD.

Hi.

Thanks for your query. The first thing I had to notice was your age: 48.

Also noted your Medical History, read and re-read the history you have given.

My inference is:
There is certainly an obstruction in the outflow.
I would request you to undergo digital examination by an Urologist to palpate the prostate. This is far more accurate in deciding the problems of prostate like fibrosis, firmness, hardness, chances of prostatitis (inflammation), malignancy etc to detect the cause of your problem.
Many times the median lobe although small can cause obstruction due to valvular mechanism during strained urination. A forceful urination can cause congestion to give similar problems.

Next step would be to undergo Cysto-urethroscopy to see the obstructive pathology and may be therapeutic as well- He can cut/ dilate the stricture if he finds.

Rare cause can be a small stone impacting at the outlet of the urinary bladder (previous ultrasound might have been normal - so I said a rare possibility and You say your check-up 2 months ago was normal).

Please get your blood pressure and sugar checked and maintain it in range.

One the perfect diagnosis is made, treatment is guided well. Also get blood PSA (prostate specific antigen) done on a safer side.
