The commonest cause of intermittent pain in the testis of a 12 year old boy exacerbated on running with other history that you have mentioned is most suggestive of the following:
Recurrent testicular torsion.
I would advise you the following:
Urgent ultrasonography and color doppler of the scrotum.
Clinical examination by a General Surgeon or an Urologist.
If this is proved then needs testicular fixation asap to avoid the torsion with its complications.
Also get the blood and urine tested to rule out any infection; if present to get treated accordingly.
Give scrotal support to elevate the testes.
Ask him to stop any movements that gives muscular action of the lower abdomen, one of the instigating factors for torsion, and anyways needs rest.

There may be mild tenderness or mild swelling.
Unrelieved torsion gives severe pain, swelling, redness, severe tenderness and referred pain in lower abdomen.
