Brief Answer:
Surgery is the treatment of choice in your case

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. Your prostate enlargement is most likely benign because PSA is 0.93 and is normal.

2. The prostate gland is 51 gm and this is moderate enlargement. Veltam contains tamsulosin and this is recommended medicine to treat prostate enlargement.

3. Due to the size of your prostate and uncomfortable bladder, it is recommended that surgery might improve your symptoms.

4. Taking veltam for 10 years is not decreasing the prostate size and therefore it is possible that it will not help. If you do not get surgery done then there might be acute urinary retention and this might be an emergency situation.

5. The result of treatment with ayurvedic medicine is not established for prostate enlargement and your symptoms will persist. However getting the surgery done is entirely your choice, but it looks like taking medicines will not help treat the problem completely.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Thanks for your reply.

My urine retention is very high. Doctor has suggested for Valtum Plus some times back. But when I checked for it in internet there were side effects mentioned. Hence I didi not started it. With valam 0.4 and ayurvedic Atleast urine flow has improved to certain extent.

For surgery pl advise whthr i should go for turp or laser. Which one is better.
Also I am afraid of surgery, since my father had Turp done and his urine became inconsistence and uncontroable. He had to were diapers always and need to change atleast 4 to 5 times per day.

Brief Answer:
Laser surgery is better than TURP these days

Detailed Answer:
Hi XXXX,
Thanks for writing back with an update.

1. I understand that you are cautious about side effects of taking medicines. Good to know that urine flow has improved however the prostate is 51 gm and that is a high value.

2. If retention of urine is there then going for laser surgery is better than TURP. Please get it done at a place where the surgeons have treated many patients.

3. Sorry to hear about the complication your father developed and presently it is rare to have such problems.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek
