Question: Hi Doctor Grief - I am experiencing uncomfortable symptoms in my genital area and near the anus. The most pronounced symptoms feel like an interior hemorrhoid of sorts and it is the most uncomfortable immediately after a bowel movement and when sitting for extended periods of time. I've used Tucks and Prep H with some mild success but typically those remedies are effective for up to 1 bowel movement. I am working on changing my diet and drinking more water, per my PCP's advise. He has not yet personally inspected the area. I have also been experiencing a seemingly related symptom on my glans penis, which is a pulsating sensation (not pleasant but not very painful) that feels like it is originating below the surface of the glans penis. Occasionally, I notice red blotches on the glans penis and on the penis side of the foreskin (I am not circumcised) though these blotches appear without any additional symptoms. I have been experiencing these symptoms irregularly for the better part of a year, sometimes without any discomfort in either area for several weeks or more. I am using a stool softening prescription now but the pulsating in the glans penis and the discomfort in the rectum area persist even as my bowel movements are less painful.

Thank you for the query.

Rectal examination is the first thing you should have. Your symptoms may be caused by hemorrhoid or anal fissure. Hemorrhoids give a lump which can sometimes be visible in the anus opening. Fissure causes strong pain when passing stool. Both are caused by constipation. Both can cause blood on the surface of the stool or on the toilet paper.
If its hemorrhoid, please note that there is no medicines which will make it disappear. Surgery is the only permanent solution. Other ways can help with the symptoms only.
Fissure can heal by its own if its acute one. After 6 weeks of fissure presence, it becomes chronic and can be treated with removal.
Your penis problem does not seem to be related with bowels movement. Most likely you are facing with some kind of infection (fungal or bacterial).
I suggest you to consult proctologist with your rectal problem and dermatologist with penis problem.

Thank you. So you do not expect that anything occurring in the rectal area would be connected to the pulsating I experience in the penis? The symptoms tend to come on simultaneously. I thought perhaps a hemorrhoid could apply pressure to a nerve or vein that could affect other areas of the body.

The only thing which is attached to the rectum and penis is prostate. So an inflammation of prostate can give symptoms of both mentioned parts. However prostate usually give sharp pain in the area between the rectum and the scrotum. Burning while urinating can be present.
In case of prostate inflammation, rectal examination should help in detecting it, that is why it should be done at first.
There is no nerve which hemorrhoid could compress and cause symptoms in that way.
