Well looking into your problem I would like to tell you that it will be very difficult to give you a diagnosis based on these symptoms but one thing I will suggest is urgent cardiac evaluation, having an ECG , ECHO . Plus a Chest X ray needs to be done for looking at any lung pathology . It could be gastritis also but needs evaluation as gastritis can be the biggest mimic of cardiac disorder .
I think you should seek medical advise at the earliest .
