What Causes Dizziness Post Intake Of Rabeprazole Sodium For Chronic Gastritis?

Question: I had constant belching and pain in chest...since oct 2014...a gastro specialist said it was chronic gastritis...after two months rebeprazole sodium course i got relief...thereafter i stopped the medicine....again in oct 2015 belching and chest pain started though not as severe as last year...i got checked for heart stress echo was normal... i am again taking rebeprazole sodium...and yes i feel diziness sometimes the whole day...i also have cervical spondilysis...i walk daily and take telemisarten 40 for bp ...do i need to consult for more cardiac tests

Hello , I am Dr.Mody and I would be addressing your concern .
I went through the history in detail . To be be precise I would enumerate my observations.

1) A stress echo cardiography is a very good test rule out any significant heart issues .
So if that is negative it means your heart is strong enough to compensate whenever needed for any physical or mental stress .

So nothing needed to further rule out cardiac problem.
However to detect some insignificant plaque

Imaging like CT coronary angiography would be the next step , here with the help of a CT scan one sees the anatomy of your heart vessel .

However this is not the reason for the symptoms you are having

2) Most probably the symptoms are due to stomach itself . By your history it's more like some reflux rather than plain gastritis .

To me there is a possibility of having gastroesophageal reflux gastritis with maybe a hiatus hernia .
Here there is reflux of stomach acid in the food pipe causing the symptoms .

So how to move furthur :

-- Rabeprazole is surely a good drug and should be taken for at least 6 weeks for healing of stomach lining . An alternative is Esomprazole ( Neksium 40 mg ) is the brand name , which is best in the group (Proton Pump inhibitors)

- a plain barium swallow would be a non invasive test to rule out hiatus hernia or reflux .
If present some permanent life style changes would be warranted to prevent recurrence .
Surgery is also an option though last .

-- it's important to rule out H. pylori infection of the stomach as it will lead to recurrent symptoms In that case , antibiotic course (Tripple regime) is needed to be taken after checking into your Gastroenterologist .

-- Dizziness can be because of reflux itself or rarely due to cervical spondolysis

-- Telmisartan is good for BP , however keep a tab on the BP as higher BP can be the cause of dizziness.
