Brief Answer:
Need to go to ER

Detailed Answer:
Hi.
Thanks for your query, albeit short and specific.

First of all, please let me tell you not to repeat the dose of Tamsulosin as it can give you postural hypotension.

Considering your age of 76, I would request you to visit the ER as there can be an obstruction that may need catherization or so.
I would advise you the following:
Go to ER
Get ultrasonography and blood tests for WBC and kidney functions.
Proper clinical evaluation, examination and further tests as may be necessary.
Catherization if needed.
Correction of other associated problems if found like correction of dehydration and so on.

I hope this answer helps you, please feel free to ask for further relevant queries if you need to or if you feel that there is a gap of communication.
