What Causes Dry, Flaky Skin On Penis After Intercourse?

Question: Hi, I was wondering if you or someone you could direct me to could help me out with an issue my boyfriend and I are having. It is a recurring issue and started about a year and a half ago. My boyfriend has told me that after intercourse, he sometimez gets dry, flaky skin on his penis...I think sometimes on the head, sometimes closer to his testicles, but it will last for days, sometimes a couple weeks.
I have been to the doctor 4 or 5 times, and sometimes they diagnosed me with a bacterial infection which I have taken Flagyl for, and just recently I went in because my boyfriend was having symptoms and they said there was no sign of an infection on my end. I have been tested for STDs, the only thing that has come up is the HSV-1 virus, which my mom has so I'm guessing I got it from her a long time ago.
We have been very good about using baby wipes, or cleaning private parts after intercourse. I take rephresh Pro-B probiotics for womens yeast and bacterial health. I also keep vagisil wipes with me. I have never had this issue with anyone, and he has never had this issue with anyone else either. What could this be?!
He says it never hurts, just is dry, flaky, and a little irritated. He has gone to the doctor a couple times, once they gave him a pill to get rid of a bacterial infection, and just recently they diagnosed him with jock itch and gave him a cream, but he says the cream seemed to irritate it even more aside from the last two days (where he has seen improvement).
I am just at a loss of whether this is something serious? None of the doctors we have been to have said it is, but i'm just not sure why it would only occur with me? I have been tested so many times since it has been an issue I just am very confused as to what it could be. If you could provide some peace of mind that would be wondertful.

Hello,
Thank you for posting on HCM.
I can understand your concern regarding the skin lesions your boy friend is having on regular basis but its difficult to point specific diagnosis in absence of clinical examination. From the description and history, I can think of either a fungal infection called candidial balopsthitis or a viral infection Herpes progenitalis.
Therefore, I insist you to post good clinical pictures to me so that I can help him in a better way.

I know this is not herpes progenitalis, he doesn't have any pain at all, it is just dry skin. There are no lesions of any kind. Unfortunately, he has not shown it to me or I would have sent a picture. He seems to be a little self conscious about it, understandably so. Given that it is not herpes, what are your other sugg
Brief Answer:
Oral and topical antifungal
Detailed Answer:
Since we are not having any clinical pictures, i would to treat him as candidial balanoposthitis, basically a fungal infection of glans and prepuce, an transmitted mosty via sexual route in adults.
I would suggest a 10 day course of oral antifungal like itraconazole 200 mg once daily.
Apply a cream containing sertaconazole or clotrimazole twice a day for 2 weeks.
Sprinkle clotrimazole dusting powder after application of the above cream.
Use ketoconazole soap for daily rinsing of genitals and avoid tight undergarments.
Avoid any unprotected sexual intercourse till complete resolution.
Also i would like him to be investigated for syphilis ( VDRL test) , HIV and his blood sugar levels.
If you have any complain of curdy whitish discharge per vagina then you too should get yourself checked by a gynaecologist or dermatologist.
Hope your boy friend will be fine soon with the treatment highlighted.
Thanks

Ok thank you very much. Out of curiosity, why do you think this could possibly be syphilis or HIV? Or possibly have something to do with blood sugar levels? Just

Detailed Answer:
Dont worry checking for HIV and Syphilis is a routine protocol for any skin lesion over genitals because the above two conditions can mimic any condition and if missed initially can have life threating implications later on.
Blood sugar levels will rule out diabetes, a condition which predisposes to recurrent fungal infections especially over gentials.
Thus its in best interest if we get all this investigations done.
