What Causes Dry Lips Along With Acne?

Question: Hello doctor,

This is xxxxxxxxx and I am battling with severe dry lips for more than two years and just before four months ago till then I used to apply Vaseline but since I had a very sensitive oily face and I was braking out I switched over to sebamed as per the doctor advise ... now for the past couple of days every now and then the lower vermillion border feels sometimes itchy sometimes warm sometimes it feels reddish ... even couple of days ago I had a small whitehead like in the right side lower chin area and it was hurting me that when I open my mouth to chew or swallow or even laugh the right side of the lower lips was hurting... I consulted a doctor in this podium and he said it looked like inflamed acne and told me to cut on oily and fat items ... now these time of sensation I am feeling on the left side of the chin area and also on the left side lower vermillion border ... I am attaching few pics for further clarity please tell me what should I do ... sometimes when I feel itchy and rub the skin ,,,, dead skin comes offf it looks more like flakiness but there is no dryness as my chin area looks very oily ... I am very much S cared and disturbed please provide me. A solution

I am Dr. Kakkar (Dermatologist).
I have gone through your query and I have noted your concern. I have viewed the images.
Pain in chin area and lower lip region could be because of a developing pimple/ pustule. Redness, dryness and flaking in this area is part of facial seborrheic dermatitis. In facial seborrheic dermatitis there can also be pus filled bumps/ pimples on chin, central face, sides of nose and glabella- which you commonly have. The central oily regions of the face and scalp (presenting as dandruff and itching) are the regions most commonly affected by seborrheic dermatitis..
For the painful spot/developing pimple on chin area you may use a topical antibiotic e.g mupirocin 2% cream or fusidic acid 2% cream.
I would suggest oral doxycycline 100mg, twice daily if you often develop these sort of pus filled bumps and pain.
You may also try using a ketoconazole cream for redness, flaking and bumps in the chin area. Ketoconazole is antiseborrheic.

Hello doctor ,

As you already know that I cannot get those prescription medicine can you please suggest some daily based creams or face wash that I can use ? Also the lower lip vermillion border itches sometimes and also I have been noticing that the line which ape rates the lip from the chin is slowly vanishing I mean the border line is not clear in the mid section ... u said about seborrheic dermatitis ... is it because I have lots of hair in the upper and lower lip area if I shave them and kept it hair free will I feel any difference .

Regarding your concern related to lip and facial seborrheic dermatitis

Detailed Answer:
Hi.

You would get an over the counter (OTC) topical antibiotic ointment/ cream e.g Neosporin or Mupirocin. You may use that for the painful spot/ pimple.
There are many ketoconazole preparations which are available OTC. You may visit a drug store near you and try to get a ketoconazole 1% lotion (Nizoral) which you may use as a face wash, though it is actually a shampoo but it can be used for facial seborrheic dermatitis.
On lip you may continue with the lip balm that most suits you.
Your lip is well demarcated from your chin skin, I mean the distinction is Ok.

Hello doctor ,

For the past two to three days I am having an unusual dryness flakiness and something crawling like sensation in my upper vermillion border , at first I thought it could be because of the dead skin accumulation hence I stubbed off the dead oily skin and applied cocunut oil as I did not have Vaseline or any other moisturiser with me then I started noticing a small pimple like eruption in the left side of the upper vermillion border and the right side is unusllly dry and feels like a tape is stuck to it . For the past couple of months this type of problem is occurring throughout the vermillion borders of both upper and lower lip ( one side or two side at a time ) and there is slots of dead oily flaky skin which goes off only after scrubbing . Please suggest is scrubbing ok or should I use some other method because only moisturiser does not remove these I am attaching few pics for clearer picture . Please let me know

Hello doctor ,
My lips are peeling terribly for the past few days I am applying sebamed every three hours and cortisone cream as per your advise ... can you tell whether I can start using cocunut oil also is eating too much dairy product cause some issues ... everyday I need to peel off the extra skin and very annoying please help

Hi.

You may apply coconut oil. No harm in that. Continue with cortisone cream. however if the lips are peeling too much you may try a slightly more potent steroid cream for a short period e.g either triamcinolone acetonide 0.1% or betamethasone valerate 0.1% cream. I think you may find one of these over the counter.
