I have gone through your query and I have deduced that you must be having balanitis.
This can be either due to dermatitis/skin irritation, bacterial or fungal infection.
It is important to determine the cause before treatment.
You should visit a dermatologist for a skin swabs and skin scrapings to test for a bacterial or fungal infection.
You should also undergo urine routine investigations to check for a urinary tract infection.
In the meantime, you need to moisturise the skin on your penis by using plain Vaseline twice a day.

During sex, I would recommend you to use lubrication.
Wear loose, cotton undergarments to avoid friction and wash the skin with a mild, moisturising soap
