What Causes Dull Pain Around Rectum And In Buttock Area Radiating To Ankle?

Question: I am a 35 year old male of asian XXXXXXX descent. For the past 10 days I have been suffering from the following symptoms
1. constant constipation
2. dull pain in the left area of the rectum
3. dull pain the left buttock which radiates down to the leg up to the ankle and sometimes up the left side of the back
4. I also found a small lump that is hard and tender to touch (not always) on the left buttock near the butt crack
5. mild stomach ache primarily on left lower abdomen

In the past my primary care physician had prescribed linzess which relieves constipation but this time it only works for a few hours and the constipation returns.

Dear Mr XXXXXX

Pain in left lower abdomen is likely related to constipation problem you are having. Chronic constipation in an adult is likely to be due to following
1. Low fibre diet
2. Decreased intake of water
3. Presence of lesion of anus and rectum

Before embarking on blind empirical treatment its advisable to undergo thorough evaluation by General Surgeon/Gastroenterlogist

I would suggest you the following

1. Plenty of oral fluids: 2-3 liter of water a day
2. High fibre diet: 2 servings of vegetable salad and fruits everyday
3. Oral Laxatives


Pain in the buttock with radiation of pain in back and lower limb, possibility of Prolapsed intervertebral disc needs to be considered. Exact diagnosis is only possible by thorough clinical examination by Physician.

Tender lump in the buttock area could be abscess, possibility of Pilonidal sinus needs to be considered especially if you have history of recurrent swelling in that area. I would suggest you to send me clinical photograph for further evaluation.
