Question: I am a 24 yrs old woman. I had protected (using a condom) intercourse on 27th Oct which was the 12th day of my cycle, as this was my fertile day and to avoid any chance of pregnancy i consumed an i-pill (emergency contraceptive) .I got my periods 12 days early on 4th November (actual date was supposed to be 13th november) . So shall i consider 4th November as day 1 of my cycle?

I have read thorugh your query in detail.
iPill contains a high dose of hormones. It is known to cause early or delayed cycles in most women for next 2 cycles. Keeping this in mind, your periods being 12 days earlier was 12 days early this time. For all practical purpose your 1st day of current cylce is 4th november 2014 but the length of the cycles might differ this time. It is best to observe the length of this cycle and next cycle and then once the duration is as it was before taking iPill, you can calculate fertile days as per your normal cycles.

Taking iPill can cause abnormal periods leading to early and delay in most women. It is also known to cause cysts in ovaries and ovarian hyperstimulation syndrome if taken regularly. It should be taken only in emergency and about once or twice a year can be allowed. The low dose pills are more suited for those who want to have unsafe sex without fear of unwanted pregnancy. With protection, condoms are the safest method of having sex.

This is my another concern, now , if i consider 4th November as the first day of my cycle, my ovulation will tentatively occur on 16th/17th. I had protected intercourse on 13th November and yesterday (yesterday was the 13th day of my cycle). We changed the condom after each act, and there were neither breakage in the condom nor leakage of the semen. As it was a fertile day, i am worried if there is still any chance of pregnancy from any remnant sperms. Should i again consume an emergency contraceptive or should i wait and get a home pregnancy test done. Are there any kits available which can detect pregnancy in the first week itself?

Your queries are important.
1. As you had sex with condoms, please do not worry. Presently most condoms of high quality with less failure rate. If used properly and a new condom each time then there is no reason to worry.

2. Please do not consume any more emergency contraceptive. As mentioned above these have lot of side effects and can cause ovarian disease like cysts and infertility.

3. Home pregnancy test is most accurate when done at least 15 days after sex. No home pregnancy test can accurately detect pregnancy before that. Blood tests are there which can detect it in first week but has to be done in laboratory.

4. If you are frightened about sex even with condoms then please apply spermicidal jelly to be applied over the condom for double protection. You as a woman can also use vaginal pessary containing spermicidal which is sold as Today in pharmacy outlets.

Can you kindly answer few more questions .

So now that i consider 4th November as the 1st day of my cycle, can i safely conclude 24th, 25th, 26th as my safe days?

How safe is vaginal pessary, is it as safe as a condom? Can one have sex (without worrying about pregnancy) without a condom if vaginal pessary is used? How to use a spermicide, and by what name is it available in XXXXXXX Also, which lubricant is safe (as in which does not cause damage to the condom) water based or oil based?

Does consuming abortion pill in early stage pregnancy cause severe pain and bleeding, or is it similar to having ones menses? Will abortion cause any serious effects in future to the chances of conceiving?

Your safe days considering that you have a 28 days cycles are between 24th, Nov 2014 and 1st, Dec 2014. This is a naturally derived estimation and so 24th, 25th and 26th November will most likely be your safe days if other factors remain unchanged. These dates are hormone dependent.

A vaginal pessary is very safe and comfortable if used as per directions. The failure rate of vaginal pessary is 4.3% at 12 months of use. If vaginal pessary is used properly then it is seen to protect even without using condoms. This is lesser than condoms which have a failure rate of 1%-30% at 1 year of use.

In XXXXXXX spermicide available is nonoxynol-9 which is marketed as TODAY pessary and sold in most medical shops. It is manufactured by Bliss pharma company. The cost is around 57 rupees for pack of 15 and 19 rupees for pack of 5 pessaries.

Lubricant recommended is KY jelly manufactured by XXXXXXX and XXXXXXX This is recommended for use with latex condoms.

Abortion and abortion pills must be avoided as much possible. It can cause irregular periods, increased bleeding for few cycles and decreased fertility with lesser chances of conceiving in the future.
