The X Ray indeed shows a broken screw as well as another displaced one. Let me tell you that the purpose of the screws is to keep the SCRAM device in place. Needless to say that the screws at the end serve the main function of fixing the device while the screws in the middle provide stability. Since the screws at the end remain undisturbed and since a number of the intermediate screws are in place the ones that are displaced or broken off are not compromising the stability. That is the reason no one seems to care. In any case, nothing needs to be done about it.
Talking about the swelling, there are two simultaneous factors contributory. The first is your heart failure. The second is the venous obstruction that the SCRAM device is causing. Together they are responsible for the swelling. Apart from the regular treatment for your cardiac failure there is nothing else that needs to be done. That is the reason that nobody seems to care.
