What Causes Elevated T4 Levels In Women With PCOS?

Question: Hello,
C/O elevated T4 level.

I am a 33 yr.old women with H/O 1.Hypothyroidism( For past 10 yrs.)
2. PCOS.(For past 2 yrs.)
For Hypothyroidism I have been taking Thyronorm 75 for past 2 yrs.I regularly check my Thyroid profile with an interval of 4 months and for past 2 yrs it remained under control.

Now I checked it today and also received the result .I shows elevated T4 ,but normal T3,TSH.

The results are....T4...13.50ug/dL Lab Normal Range (5.01-12.45)
T3...1.08ng/dL Lab Normal Range(0.60-1.81)
TSH...3.48 ulU/mL Lab Normal Range(0.35-5.50)

I would like to know how would you interpret this result? Is there any dose alteration necessary?

I also checked Blood R/E,Blood Sugar and found them as normal.I don't have any weight control issue and the only symptom of my Hypothyroidism is excess hair fall for which I regularly consult with Dermatologist.In a word ,for past 2 yrs I have been dealing pretty good with Hypothyroidism, until today. My today's Thyroid profile result with elevated T4 actually prompted me to contact you.

Ma'm, you have mentioned that you have PCOD from past two years, so are you on treatment? It is a possibility that total T4 , that is what you have checked , may be falsely high if you are on some other medications, or may be due to some other reasons. But as your T3 and TSH are normal, definitely the dose you are taking is the right dose for you. kindly confirm it with FT4, which will give you the correct status of yoour thyroid profile. for Hair fall, kindly add more of iron, protein and calcium in your diet.

Hello Dr. XXXXXXX

First of all thank you for your valuable time and advices.

Now,come to your query,regarding the PCOS and its treatment.It's so righteous for you to doubt and eliminate any secondary cause behind my illness.

Mam,myself is also a doctor (B.A.M.S and also currently a 1st yr M.B.B.S student),so you can trust me that no disease would be untreated ,once it was diagnosed.

So,for PCOS ,I am under treatment of a reputed Gynaecologist and I have been taking Dronis 20 for past 2 yrs.I visit him with a regular interval of 3 months and according to him I am doing good with my PCOS.

Now,come to the recent elevated T4,what you have indicated of a false report ,might be very possible and justified.

But I would request you to kindly check out my previous Thyroid Profile,which I did on last December'14(7 months ago). You will find that T4 was within the normal range,but slightly inclined to the higher note(10.50 ug/dL ,while the normal range was 5.01-12.45).
I also need to mention you that for past 2 yrs. I have been taking Iodine Supplement too(Pls,check the uploaded image).

I should give you the total list of medicines,I have been having daily.......
1. Thyronorm 75mg
2.Vitamin shoppe Thyroid Complex 2 cap daily(Supplement)
3.Dronis 20 daily
4. Saw Palmetto 320mg daily(Supplement)
5.Biotin 10mg daily
6. Minoxidil 2% topically.
7. Multivitamin ,specially for hair .

Since you have expressed concerned about the protein,iron,calcium of my diet,I wish to ensure you that I follow my diet strictly which has been designed and revised regularly buy a professional dietician.

Doctor,I have arranged a freeT4 test tomorrow morning.I want to know that should I take my Thyronorm dose as usual before the test or refrain from taking it till the test?Pls,reply.

Dear ma'm, if you are giving the blood sample early morning empty stomach, then skip the dose, if you are giving it some other time of the day, then you can continue with your routine dose.

Hello ,

Dear Dr XXXXXXX as per your instruction, I did Free T4 test yesterday and today I received the report which came as normal.

The results are.....Free T4..1.33ng/dL (Lab normal range..0.89-1.76)
Free T3..1.01ng/mL (Lab normal range..0.60-1.61).

(I have also uploaded the scan copy ot the report ).

So,please share your thoughts regarding my problem,which I doubted as thyroid related excess hair fall.Please,let me know what you think of my current dose of Thyronorm(75mcg).Will there be any necessary dose alteration?

Dear ma'm, as already discussed , Total T4 was high but free T4 is normal. so kindly continue the same dose of thyroxine tablet. For Hair fall continue high protein and iron intake. Take supplements if needed.
