What Causes Elevated WBC Count After Protected Sex?

Question: Dear Doctor,
I had a low risk exposure on 5 May 17; penile masturbation by a CSW with her clothes ON; as she slid condom, kissed here twice near her lips; fondled naked breatss; I ejaculated in condom; no intercourse; tested negative for hiv1 and 2 on 6th day , p24: 0.02pg/ml on 14th day; on 28th day, tested negative for hiv 1 and 2; p24: 0.01 pg/ml--negative; developed diarrhea from 25th day : 3 times a day; blood report says spme bacteria present and wbc count is 10100; IS THIS P24 RESULTS OK?? and what about the WBC count? what test for the bacteria?

am I ok? do I require further testing? why this diarrhea? currently on liver dosages

I have gone carefully through your query and understand your concerns. The p24 report is negative and therefore ok. The WBC count is slightly raised and I would need to ask you the differential count (neutrophils in particular). Possibly you have a bacterial gastrointestinal infection. No specific test is needed for 'bacteria'. No further testing is needed. You need to take ORS (oral rehydration solution) to make up for the lost water and salts. In a few days you would be fine.

thanks for your reply

(I)differential WBC count
(a)Polymorphs: 76%
(b)Lympocytes : 20%
(3)Eosinophils: 2%
(4)Monocytes: 2%
(5)Basophils: 0%

Is my current medication ok sir? will take ORS. experience stomach pain; local physician says my immunity to be improved further B12 low.

Serum B12 level: 243 pg/
absolute CD4 count: 850 /cumm

pls advise whether I have to take any hiv related test??
and for this diarrhea related, pls advise
shall I close this hiv query or have to wait further. pls advise on tests, treatments allopathy/ayurvedic as u feel

pls help me do right treatment further; want to take the best treatment at earliest. plsplsps advise sir

Test not for HIV but for diarrhea

Detailed Answer:
Thanks for writing back. A rise in the neutrophil count and the total WBC count (technically called neutrophilic leucocytosis) is an indication of infection (gastrointestinal infection in your case). Antibiotics like ofloxacin can be taken but are not necessary and irrespective of antibiotics it should recover in a few days. You can take probiotics like Enterogermina one such twice daily for next 5 days.

The test is not HIV related and is for diarrhea. No further testing is needed. No other medicine is needed.
