What Causes Elevated Fasting Blood Sugar Levels?

Question: I am diabetic. my sugar level is 180-190 in the morning fasting. however is 130 or 140 during afternoon i.e. pp. I am taking Glycinorm -M 30 OD in the morning after breakfast, can you tell the reason for high sugar level in the morning. I exercise regularly 35 to 40 minutes in the morning. diet is fine not very strict.

Thanks for your question.

After going through your question, I can understand your concern.

Fasting blood sugar depends upon hepatic glucose out put.
Hepatic glucose out put means production of glucose by liver to fulfill calories requirement of body.
It depends upon basal insulin secretion that is insulin release during fasting state.

Type 2 diabetes is due to insulin resistance at the level of liver and peripheral tissues.

When insulin resistance at liver is more than peripheral tissues fasting blood sugar level is higher than post prandial as in your case.

I would recommend you to continue Tab Glycinorm M 30 as before and add Tab containing Metformin 1000mg after dinner (considering normal kidney and heart status)
Try to follow diabetic diet strictly and keep following regular exercise.

Recheck your blood sugar level (Fasting & PP) after 3 to 4 weeks and depending upon it appropriate changes in medicines can be made.

All this should be discussed with your treating doctor.

Additionally following investigations may help in appropriate management of diabetes and its complications-
- Lipid profile.
- HbA1c.
- Renal and Liver function test.
- T3, T4, TSH.
- ECG and 2DEcho.
- Screening for diabetic complications.

Dear Dr. Abhay XXXXXXX

Thank you for your reply. I want to provide additional details . I am hypertensive ( taking ATEN 50 ) and had a angioplasty for 80 % block in 2012. The other medicines I take are Clopilat A 75, Telsartan 20Mg, Storvas 20. Yesterday I visited my Doctor and she has increased my dosage to Glycinorm -M 60 /500 . one tablet in morning. Doctor has told to take the changes medicine for 15 days and she will review it. I want to know if I take Dinner early or reduce my dinner portion, will it reduce my fasting sugar ?

I have already mentioned fasting blood sugar mainly depends upon hepatic glucose out put (glucose production by liver in the absence of calories intake).

There is some effect of calorie consumption in dinner on fasting blood glucose level.

Hence there may be some effect in reducing fasting blood glucose level of your proposed change in dinner.

Rest you are on appropriate medicines for your blood pressure and heart problem.
