Get checked for Filarial scrotum.

Hello XXXX and welcome to HCM.
If the enlargement is recent, you need to see your doctor for finding the cause for it.
If it's been there for last few years, there's no urgency at all, unless it causes pain or you notice recent increase in size.
This kind of enlargement of scrotum is seen in Filarialiasis.
What you should do before you see the doctor is, get your blood counts , includuing eosinophil, ESR, sugar.Do an Ultrasound scan with doppler of the scrotum and see your doctor.

Yes, the swelling happen overnight but no pain

If the swelling appeared overnight, you need to see your doctor and get a scrotal doppler scan done.
