What Causes Excessive Burping Upon Waking Up?

Question: PROBLEMS PERTAINING TO DIGESTION AND HEART BURN
1)     Every morning after waking up, burping starts at empty stomach, followed by feeling heavy around the chest.
2)     Stomach doesn't empty properly, Irritable bowel, Bloating, belching and gas.
3)     Even after having a light snack or tea, burping occurs and feels like chest area is full,
4)     Fullness during or after a meal
5)     Acid Reflux, if eaten spicy food. Acidic taste in your mouth
6)     Feels like food is moving to Esophagus
7)     Gallbladder operation in 2005, wherein the bladder was removed along with the stones
8)     As prescribed by doctors have been taking Antacids for past three years but result is not prominent. I am not fully recovered from this issue. The day I don’t take medicine, problem incurs.
9)     Doctor at AIG given PH monitoring test, but could not do that as the pipe was choking while it was inserted through my food pipe.
10)     Frequently have to go to toilet, mostly after meals.

PROBLEM PERTAINING TO RESPIRATION AND PALPITATION
1)     Suddenly heart palpitation with chest fullness takes place followed by shortness of breath and rapid heart rate. Even it occurs some times after eating. A mild physical workout or running also results in the same sometime.
2)     An irregular headache followed by neck pain and dizziness occurs during then.
3)     Doctor at Tagore Hospital XXXXXXX advised for PFT test but did not do it till now.
4)     Respiratory issues
SLEEPING DISORDER
1)     Not having enough sleep, there is a feeling that something is in between airway passage (sometime). Spent many sleepless nights, maximum time that I could sleep in a day is maximum 4 hours.
2)     Suffer from emotional issues, feels too much tired and down. Feels depressed.
3)     Too much weakness
HEART RELATED ISSUE
1)     Was diagnosed with acute MI during an ECG undertaken in the year 2012, shown many doctors, done several tests at XXXXXXX Mallapudi Hospital but heart condition was fine. Doctors said that once this has been diagnosed, every time an ECG will be done, it will show the same thing. But nothing could be found in Echo 2D, treadmill test, Blood protein test.
2)     Medicines taken – Ceruvin, Storvas 20, Tonact 20 but never continued for a long. There was always a stop gap of two to three months. Most doctors advised for life style change. Stopped smoking and drinking completely.
3)     LDL was high for some time, but after proper medication and exercise, it was also reduced to normal.
BONE / MUSCLE RELATED
1)     Joint pain, full body pain.
2)     Neck pain, muscle pain
3)     Lower back pain
4)     Was diagnosed with Vitamin D deficiency in 2013 , but after proper medication it came to normal.
SKIN ISSUES
1)     There is a peculiar type of skin infection takes place during May-June –July and again during the middle of winter Dec-Jan. Rashes red in colour comes out at certain locations with itching sensation.
2)     Burning sensation followed by pain in right palm for last 2 days.
WHAT I DO
1)     I do heavy weight training 4 days a week.
2)     Have run 6 10K runs officially in events across XXXXXXX
3)     Completed 2 half marathon last year
4)     Being Member of XXXXXXX Runners club, participate in Sunday long run almost every third week.
5)     I take MULTIVITAMIN capsules one each day Brand-Optimum Nutrition
6)     Weigh Protein- 4 times in a week Brand – Optimum Nutrition
7)     BCAA (Branched Chain Amino Acids) – 4 times in a week- Optimum Nutrition
8)     CREATINE – 4 times in a week- Optimum Nutrition
9)     VITAMIN-C capsules 5 times in a week (Dose-1 Capsule)

I have gone very carefully through your query. Your problem seems to be due to gastritis or gastroesophageal reflux. Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets hampered, food along with the digestive juices would stagnate, stomach acid will reflux into the food pipe and on the other hand the bowel habits would not be proper. The mentioned respiratory problems are likely to be a sequel of the reflux. The headache could be related to stress which could be contributory to the increased acid secretion.

​​In order to relieve yourself, you need to do the following:
* Avoid spicy food. Take small but frequent meals. Avoid long gaps between meals.
* Avoid alcohol intake (if it a part of your habit).
* Take Prilosec (omeprazole) once daily before breakfast.
* Take plenty of green leafy vegetables as well as other sources of dietary fibers. Take fiber supplements like isabgol/psyllium husk or senna. I would insist you to increase your dietary fibre intake.
* Regular morning walk and exercise to keep the bowel movements proper.
* If I had a patient like you I would perhaps have suggested Prodep 20 mg tablet once daily for at least 2 months (it may take time for the action to set in).
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.

For the low back pain you need to get a X ray of lumbosacral spine (AP and lateral) done and based on the findings further suggestions can be given. A physical therapist would be able to guide you with the right exercises that would help you. Hot compress should help.

Thank you Sir, hope that you have also read my blood report. My testosterone levels is in border line and C Reactive protien is 3.98, iron is 54.1 and total cholesterol is 205. I have been suggested by a doctor here in XXXXXXX to take pantodac d, uprise D3 as vitamin D is less. In case of pain doloneuron tablet. He hasn't given me anything for iron deficiency and for testosterone deficiency. I am feeling too much weak and lethargic for more than a week, can't even go to gym or do any kind of physical work out and also undergoing through a severe headache for almost 24 hours. My blood pressure is also fluctuating from 140/90 to 135/80 yesterday and today 130/90. Sir I am too much worried about any hidden disease. Gastric issue is something which is actually ruining my life. please advice what has to be taken to overcome this weakness and to gain lean muscle. Sir do you really feel that I need a anti depressant, is so than why it has to be taken and when. Please advice.

I have gone through the reports. Your testosterone level is normal and nothing to be concerned about. CRP can be raised due to some inflammatory or infective cause. Clinical evaluation and correlation would be needed to know why it is raised. The Vitamin D levels are normal at present and so you need not worry. You need to bring down the salt intake in order to control your blood pressure.

Worrying does not do any good. Worrying and stress in fact make you more vulnerable to diseases. It essentially is the cause of your gastric problems. So my suggestion to you would be to relax. Yoga and meditation would help. Prodep (fluoxetine) is also an antidepressant and so it has dual action. Taking it should help. You may feel free to discuss about it with your treating physician.

Sir, good morning . Thank you for being so kind and making me feel positive about my self. Sir, how could I get my CRP further investigated, kindly suggest because I am suffering from rashes all over my hand and other body parts time and again. Please suggest when Prodep to be taken. Also, please guide me if I need to take any other medicines. I am practicing yoga in the morning followed by a walk. Can I run marathon on 28th August or should I cancel the event for now. Sir, i understand that my Testosterone levels are not as much as it should be.
I don't have much desire for sex. Like ED, also i am not having diabetes, then why there is low sexual desire.
i am feeling the effects in many areas of life. my energy levels have gone down, muscle strength has droped. also there is a substantial change of mood, sometimes low followed by fewer and weaker erections. I am really worried.
As i had already mentioned that i have been doing weight training and running , now i dont feel like hiting the gym or dont find that boost in running. Lacking Endurance.

Yes i believe that i will surely try my level best to follow your instructions pertaining to improvement of Sexual life.

Yes, I have gone through the prescription.

1. If your sleep is disturbed, you need to reduce the stress and anxiety levels. Your questions reveal your anxious nature, something that you need to deal with. Without it the condition would not improve. Meditation should help but that is not all. The main thing is your approach toward life. Things going wrong is an essential part of life. Take things as they come. Do not keep dying in apprehension that things would go wrong. When things deviate from normal, it does not always mean that you need to worry about it or become panicky. Reducing anxiety provides a better grasp over things and therefore is better in bringing things back to normal. Same about sleep. Prodep is an effective anxiolytic and should help. Other sleep inducers may be taken but all are prescription medicines and you would need to get them written down by your doctor. Zolpidem is a very good medicine in this regard.

2. I would not suggest you to take any medicine for your male hormone issue. Lifestyle modification is the primary thing here. Stop consider reports being the real version of you. Bring out the real you and you would be fine.
