Question: Hello,
Two weeks back, I was going to my office. I went to my office stayed there for 5 minutes and came out. While coming out I saw a little scratch on my hand and little blood. The scratch was like two pin prics were there very close by. After sometime I started thinking may be some dog bite me while going to office and this worry grew and after one week I started taking rabies vaccine. Two days back I felt like I am having a lot of salivation in my mouth and yesterday I had neck pain on behind. I keep on thinking about rabies and feel differently while eating and is really scared. Because of this I had lost my apetite too. I met doctor today and he tells its just a panic attack. But still I am not getting over it. There was no pain or tingling at scratch site but from today I am pressing it continuously to see if pain is there and sometime while pressing I feel pain is there. Please guide me

You have seen scratch mark over hands and then felt that you have got dog bite and started having anxiety about Rabies and wanted to get vaccinated for Rabies. First of all I want to reassure you that you are aware of the fact that you are not bitten by any animal and these scratches were due to some minor injury only, so there should be no fear of Rabies. If you are infected with Rabies then you must have gotten dog bite, but you didn't get dog bite.

These symptoms are occurring due to anxiety only. You are aware of the thing that you didn't get any animal bite still you are having fear from Rabies and these are symptoms of severe anxiety and panic only. To some extent these symptoms are related to obsessive thoughts though detailed evaluation is needed.

Obsessive thoughts are recurrent intrusive thoughts that dominate individuals mind despite individual's awareness of the fact that these thoughts are useless and they produce significant distress. Severe health anxiety can also cause such symptoms. The checking to feel tenderness or pain again and again has become like a compulsive behaviour and this is occurring probably due to anxiety only.

I would advise you to try to reassure yourself that this is not a Rabies infection. You are absolutely normal and have got no infection. Try to relax yourself by relaxation exercises and keep anxiety away. If you are not able to control these thoughts then please visit a psychiatrist. Mild doses of anti anxiety drugs like SSRIs as Fluoxetine, Sertraline for some days would result in reduction of these symptoms and you will feel better. Please consult a psychiatrist for severe anxiety and panic symptoms.

Hello Doc,
Doctor has advised me take Clonotril 0.5 mg. I am feeling little better but still not completely over it. While eating I have vomiting feeling and it increases once I start noticing whether I am able to swallow without difficulty or not. Needs your suggestion. After eating also I have vomiting sensation for a while.

Hello again

Clonotril or Clonazepam is a benzodiazepine drug and the drug can be used to control symptoms of severe anxiety and panic and this is the probable reason you have been advised to take that drug. The symptoms like you are facing difficulty in swallowing and ideas whether you would swallow or not, nauseating feeling etc are due to anxiety only. Please try to assure yourself that you have not bitten by any animal then how could you get infection from Rabies. Try to accept that the scratch marks were of injury only and not due to animal bite. This is anxiety only and would reduce with Clonazepam.
