                                            Question : I cant seem to get motivated all I want to do is sleep, don't want to go anywhere, could care less if I ever leave my home. I have absolutely no energy what so ever. I take Prozac and seriqiul. It's been 10 years and I think it's never been worse. Please tell me what I can tell my doctor. Thank you so very much

I feel there is poor response to current medication and there is need to try alternative treatment for depression. You just need to tell all these symptoms and request him to change treatment. This is common with SSRI to stop doing its work on long term use which can be resolved on changing to other antidepressant.

You need to discuss the current response with medication. On this he will advice you the alternative medications. The alternative may be SSRI but in case of failure of SSRI SNRI are generally preferred (as in your case).
Secondly you also need to tell him what kind of medication you want and what sort of medicines need to be avoided.
Thirdly you need to inform the past response as well as first episode symptoms so he can take informed decision for your betterment.
