Just went for angiogram to look at my heart and my stent that have been put in last year. Everything looks ok. my last A1C is 5.2. The weather here has been extremely hot, about 100 and dry. Brief Answer:
Losing extra water has resulted in dehydration causing the symptoms

Detailed Answer:
Hello XXXX, I am Dr. Mody and I will be addressing your concern,
I went through the history you had provided to all the previous doctors.
Considering your stent is working good and also there was no significant ischaemia on the previous study, I would like to consider that tiredness is not much because of heart.
Now the increase in urine inspite of less water intake and such hot climate is something we need to figure.
A possibility here is you have been put on diuretic, a commonly used medication when mitral regurgitation is present.
If you can send me latest reports and complete medication list you are taking I would be in a better position to help.
Till than I request you to hydrate yourself as losing the extra amount of water might be one of the reason you are feeling so weak.
2nd possibility is over treatment with thyroid medication, because previously too one report suggested very low Tsh, which means you are taking higher thyroxine than needed.
The vision blurring if recent, is mostly because you are dehydrated because of losing extra amount of water.
If some component of blurring in vision is more than 6 months a retinal examination for knowing long term effects of hypertension on your eyes, as the same time rule out cataract.
Your hba1c is well below normal and we are not worried about the same.
Lastly rule out local cause of increased urination like. Prolapse or as simple as urinary infection
Hoping I could help.

Brief Answer:
Take water with electrolytes.

Detailed Answer:
Hello,
You need to continue hydration, as you are not on any diuretic, mostly your heart pumping should be good. So you may add electral powder to the water to keep the strength as we would need to supplement electrolytes also with water.
Again at the time when your Tsh was very low you were still taking synroid 88 mg, which I think you should review as hyperthyroidism may be the reason of many of the generalised symptoms you have.
Also why I am saying the above it, because doxepin can cause severe side effects in hyperthyroid state.
Otherwise cardiac medication clopidogrel and carvedilol will not cause the symptoms you describe.
Anyways you do send me the blood investigations and angiography report.
Wishing you best of health.
