Question : I am a 34 year old male and i am experiencing extreme pain, burning in my left nipple. Visible changes, worried about pagets. Dr prescribed cortozone cream.

To recapitulate: Male/34 - extreme pain and burning left nipple - visible changes - worried about Paget's - Doctor prescribed cortozone cream ...

Please give additional information:
- Since when have you got this problem?
- Any other medication have you take in view of symptoms you have?
- Is there local raised temperature?
- Any nodes in the armpit/axilla?
- How was it started - any minor/trivial trauma or on its own?
- Any other information you would like to share?

My thoughts:
Looking at the pictures and considering your history of of extreme pain and burning, I would think of an infection as the most probable cause if this is present for short duration.
- A course of an antibiotic, anti-inflammatory medicines should help to get a relief
- If no relief then further tests should be done:
High resolution ultrasound and color doppler.
FNAC
Biopsy to get the exact diagnosis and to get a plan for further treatment if required.

Its been a week, no meds yet, general mild discomfort and flushing of left breast/pectoral region, doctor found no lumps/nodes, no apparent trauma

Increased redness and swelling.

Detailed Answer:
The third picture shows an obvious infection with increase in swelling and the redness.
I think you need to have an antibiotic and an anti-inflammatory medicines.
