Question: Hi Doctor
My dad is around 64 year old and just last week , we did proactive health check for thyroid, uric acid, cbc and sugar fasting. Reports are attached for your review. I can see mostly all are ok but he always feel extreme weakness, constipation and lack of sleep most of time.
May i request you to prescribe some health supplement required at such age(recovit, zincovit,shellcal,liv52, biflac etc).he is pure veg and his eating pattern is not very good(one chapati + lil fruit thrice a day).
He takes a glass of milk only once a day. He is happy to take supplement as you prescribed.

Brief Answer:
No further supplements necessary.

Detailed Answer:
Hi XXXXXXX
I am Dr. Prakash HM and I would like to help you.
From the reports -
1. T3 levels are a bit reduced. - require close followup after 6 months as TSH is normal.
2. Hemoglobin is bit low along with raised MCV indicating Vit B12 and folic acid deficiency
3. Most importantly Urine is showing increased pus cells. He has urinary tract infection and he will require treatment with antibiotics. You have to get an urine culture and sensitivity to confirm and treat urinary tract infection.

He can continue the supplements of becosules and shelcal everyday once preferably in mornings.

Other thing he needs to get more is SLEEP.
SLEEP could be the cause of weakness and constipation.
He has to maintain a sleep hygiene. Disturbed sleep leads to abnormal eating patterns, gastro-intestinal disturbances, weakness, day sleeping, extreme tiredness and mood changes

Sleep hygiene is essential unless he has some sleep disorders. He can follow:
1. Avoid going to bed, when not sleepy
2. Warm bath and light snack before bed. Avoid watching TV or talking on phone during bed time
3. Wake up at same time in morning
4. Avoid frequent naps during day. No naps after 3 pm.
5. Avoid coffee, large meals or any excitement during bed time

He should also follow
1. Regular morning walk
2. Resistance training and strength training

He can take any supplements. As all his levels are near normal, he can take the above supplements. Once his sleep pattern improves, his diet should improve. He can take more fruits and vegetables. Too many supplements do not serve any purpose other than burdening the person to think of himself as a patient. If his diet does not improve, then you can give him any supplement you want.

Hope it helps

Regarding sleep, he dont watch much tv, go for regular walk but still sleep is not improving. Can we give himalayas ashavganda or anxit. 0.5 for few days?

Hi XXXXXXX

He can take probiotic for a week to help him with his constipation. He can also take Riconia G or revital capsules along with becosules for improvement of nutrition.

Himalaya ashwagandha is supposed to be beneficial to reduce stress. He can take it until his sleep and weakness improves. Also try to improve his sleep hygiene and diet.

Revital contains gensing which may cause anxiety and not good for heart of older people. May i ask you about zincovit or recovit ? For folic deficiency , is it ok to take folvite tablets? Ofcpurse for XXXXXXX 2 weeks as longer time can cause sideeffect.

Brief Answer:
Take Zincovit and later Becosules

Detailed Answer:
Hi XXXXXXX

Gensing is not known to cause anxiety. If you are worried about it, we can remove it and it would be better to take zincovit compared to recovit. Zincovit also contains folic acid, so you need not take additional folvite tablets.

Taking vitamins in excess can be only toxic with Vit A,E,D and K.

After 1 month, you can go for Becosules as it does not have any side effect or toxic effects. Nutritional deficiency of Vit B12 and folic acid is common in elderly and it contains both.

Any further queries, do ask again.
