What Causes Fatigue And Burning Sensation On Foot?
Question: Hi Doctor,
I am writing in detail please try to figure out the illness/issue what I am having,

I had a sex with call girl with condom in 17 April 2013, this activity took place for just 10 mins(until mind came into reality).
3 months after( 22 july 2013) I got flu, fever which reduced in a week but dry cough persist for almost 4 weeks.
I just neglected by taking fever tablets, thinking nothing would have happened since I used condom.
symptoms increased to multiple as follows:

1) Dry cough
2) Weakness, feel no energy to stand, yellow color urine for a month
3) mouth ulcers , random chills & trimmers.
4) Mild pain on anus and feels to go for loo multiple times( done ultra sound but results was normal)
I rushed to the Hospital with fear & get tested for STDs & routine boold tests ( God grace all came negative)
a) HIV 1 &2 ( tests repeated at 4 month, 7th month, 1 year, tested both XXXXXXX & western blot), VDRL, Chlamydia, Herpes Igg/IGM, syphilis Igg/IGM, Heptitas AB &C( repeated 6 & 12 Months)
b) In mount and penis I got Fordyace bumps( In mount biopsy done by ENT at XXXXXXX XXXXXXX hospital & confirmed it is fordyce)
c) Tested CBC & urine routine , sugar test, Vitman d & B12 etc
Though all the above test results are negative & this has happened 3 years back ,
Current situation, I am still suffering with the below things:

1) Regularly I'm getting flue & fatigue (2 to 3 months once)- past 3 years
2) Mild skin burning sensation on the foot(above part of the foot) and some parts on the hand( not daily some times, when exposed to stress and sunlight)
3) Frequent phlegm( pale yellow ,rarely black), little difficult to swallow due this phlegm/thickness of salavia- past 3 years- this is big issue Currently facing
4) I got Dry eyes (itches some times)- past 2 years
5) Feels Itchy back of ear -past 6 month- (this is irritating some times)
6) Feels Itchy on hand and become swelling immediately and after 2 hours it reduces by itself- past 8 months- got reduced after taking pills given by Dermo for allergy
7) Get cough if I eat Brinjal(garden egg from eggplant)- past 8 months.
8) Digestion problems( can't eat spicy food) and also feel sudden hungry( sugar test is normal) , also mild burning pain on tip of anus if I sat down on( no boils or abnormality)
9) Dry skin

From the above Issues I don't have any clue how to proceed further with my health, Is it an STD of some kind (or) autoimmune disease (or) allergy? or any other decrease.
Sir, Please help me with your detailed analysis if possible corner the issue I am facing by consulting your colleagues & co-doctors
If at all If I need to be diagnose, what kind of test I need to taking? as I am In deadlock how to proceed and what to do whom can I approach ?

Thanks for posting in HCM.
I went through your query also went through all the reports.
Mainly it doesn't look like a std. So that could be ruled out as you have already tested negative for hiv, vdrl and others.
The symptoms you have mentioned are very vague and very difficult to arrive or pin point to a diagnosis.
All other reports are normal except vit D is low but that won't explain the other symptoms.
But still if concentrated on the symptoms you mentioned of dryness of eyes and thick saliva and phlegm I am thinking more in terms of sjogren's syndrome. Not sure but sounds like it.
It can be diagnosed by doing sjogren profile.
Serum ANA, SSA and SSB can be done to rule out sjogrens.
If ANA comes positive then we might need to see in detail about other syndromes.

Are you sure about the syndrome you are pointing, anyways I will do test for serum as you said.
Please let me know sequence of the test , you want me to do Ana alone based on the results SSA& ssb?
When I go through about sjorgen syndrome in net, it specifying causes as t-cell virus, is it so? Is it sexually transmitted desease?
My symptoms as I told,

I am not arriving at a diagnosis of sjogren's. I am suggesting it could be a diffrential diagnosis for all the complaints you have.
As it is you have done almost all the tests available but still we are not able to arrive at anything.
Your absolute eosinophil count is in normal range so I do not think you have any allergic problem going on. If it would been allergic your eosinophil count would be raised which is not what report says.
And sjogren is not a std. You remove the fear of std from your mind.
It is not caused by viruses but some studies say it may cause. Mostly its a connective tissue disorder.
But to say it we should have some evidence hence I feel that get an ANA, anti SSA/ro and anti SSB done. Get back with me with the reports and we will take it further from there.
No need of phlegm culture or anything. It doesn't look allergic based on your blood report.
