Question: Sir, i face the following situation everyday-
Wake up tired with body ache, dread going to office, at office hiding away from face to face conversation with colleague. I often go and sit alone in car to relieve anxiety. When someone is explaining something i get lost and can't keep track. Also Im intolerant to heat(summer) and develop anxiety when exposed to hot environment ( as per my interpretation).
Also I did consult a psychiatrist and he diagnosed MDD, OCD. Prescribed Fluoxetine 40 - 2times, Fluvox 100-2 times, Admenta-1 time, Apriprazaloe 1 time at night.
Also my sleep pattern is disturbed and wake up often with dry throat .
I also take meds for pressure, diabetes.
All the above symptoms is troubling my career and always find excuse to skip office. I think I should resign from job.

Detailed Answer:

Hello,

From the available description there is enough evidence to suggest that you are suffering with MDD but none of the above symptoms indicate OCD as diagnosed by your doctor. It is possible that during in person consultation he got symptoms and signs of OCD and that lead to diagnosis of OCD.

I also feel that medication need to be changed provided there is limited response with prescribed medication despite 2-3 weeks of continuing medication. This is because both of antidepressants (fluoxetine and fluvoxamine) belong to SSRI category and if they are not helping you it is time to add/change to SNRI or novel antidepressants.

In similar situations I prefer to add mirtazapine provided there are enough indications.

I hope this helps you. Feel free to write back to me if you have more questions.

Sir, I have attached/uploaded my psychometric assessment.
Moreover I have observed that I can't talk face to face with people. Also think that something abnormal is with me. Anxiety symptoms appear every now and then. Even before driving and sometimes going to office I have to take Clonazepam which makes me dizzy and I feel hyper aware of this dizziness that makes me again anxious. This cycle repeats 3-4 times. Pls suggest a remedy

Detailed Answer:

Hello,

I have gone through the attached details of psychometric assessment and feel that you got the right diagnosis.

In the similar scenario I often recommend Flupentixol plus melitracen as day time anxiolytic as they are less sedative in compare to clonazepam. Since it is prescription only medication you will need a proper prescription to procure medication from pharmacy.

I hope this helps you further. Thanks.
