Question : A patient presents with fatigue, aching joints, sore throat, feeling "flu like" for a week or so. Pt has swollen lymph nodes and elevated temp. Mono spot test negative. WBC 2.0.... Is this mono?
The patient returned to FNP office the next day with elevated temp, increased pain from sore throat, generalized aching. IGM less than 1.0, IgG 1.2, do you need other labs?

Based on these lab data and symptoms, I think that this may be mononucleosis and I think that patient is now on the phase of the conversion of IgG.
I'd suggest doing an abdominal ultrasound as well to see the size of spleen and liver.
