What Causes Fatigue, Weight Loss And Swollen Lymph Nodes In The Groin?

Question: I have been having extreme fatigue, generalized weakness, weight loss and enlarged lymph nodes is my groin and cervical chain. Had a normal cbc and sed rate- kidney functions are off as they have been for a year- creatinine at 1.3 with a GFR of 44. had a CT that determined that my lymph nodes looked okay-although enlarged were small with a fatty hylum. My symptoms are getting worse and I dont know where to turn. Any suggestions?

To recapitulate: Female/47 - extreme fatigue, generalized weakness, weight loss - LNs in groin and neck - normal CBC and ESR - Creat at 1.3 and GFR of 44 - CT shows mild enlargement of LNs with fatty hilum - getting worse and does not know where to turn - consulted a Doctor - in need for suggestions:

The reasons which I can think of for your symptoms as per the history you have provided are:

Kidney disease can be one of the reasons as suggested by less than 60 range of GFR.

Although CT scan has shown small lymph nodes (LN), there should be FNACs that is multiple fine needle aspiration cytology or a formal biopsy of the largest lymph nodes to get a histopathology reports. This is necessary in view of the symptoms that you have, groin lymph nodes are rare to enlarge and CT is just an indicative study - only FNAC and/or biopsy are confirmatory.

Since CBC and ESR are normal, they can not predict the cause.

Other causes and advises to be considered are:
- Anxiety/stress, please tell your Doctor about this and get treated if present.
- Nephrologist's opinion for kidney problems.

Please give additional information:

- Since when are the symptoms present?
- Any associated problems or history related to any of the alterations that you or your family and friends have noticed?
- Appetite/sleep/ urination and bowel habits?
- Tests for vitamin deficiency, particularly Vitamin D and calcium, electrolyte balance.
