Posted on Fri, 4 Apr 2014
Question: Im experiencing the following symptoms:- Extreme lethargy and tiredness without doing anything at all, at times feverish. Stiffness in body especially palms and joints. Loss of appetite due to worrying about health. Loss of interest and energy in day to day activities. Eyes are worn out whole day (like the feeling of sitting in front of lcd screen and the warmth in eyes felt thereafter) Difficulty in co-ordinated movements of body Confusion and blankness (even shows up in face) slowness in thinking Takes time to register what opposite person is talking Tingling in body (right side) Right side of head and body feels different and I've suddenly, become very conscious of this. Overwhelming feeling in head (like a state of too much action happening around) and is very difficult to talk and concentrate at such times. Short tremors in right side of body, they are occasional, mainly on index finger and mainly when resting. Some short vibrations in calf not visible externally. Tendency to jerk right side of body, at times quick jerky, movements which are not intentional Slowness in speaking and framing sentences. Hot flushes (ears turn red) and head warms up Fear of imbalance when walking. Current medication 5 shots of meganeuron taken 2 shots of rabipur taken (didn't take more since the pet dog is found to be ok and non infected) 1 cap c-bone daily since 1 month 1 tab ciplar-la 80 daily 1 tab amtas-e daily at times goofing up words when talking at normal pace, hence speed of talking reduced

Brief Answer: My reply is below. Detailed Answer: Hi Ms XXXXXXX Thank you for posting your query. I appreciate the details posted by you, as it helps us more in understanding the problem. Have any investigations been done for you? We are interested in the following: 1. Serum vitamin B12, Serum vitamin D level, 2. Thyroid profile, 3. Hemoglobin 4. Blood sugar 5. Blood cholesterol 6. Homocysteine level, 7. MRI brain. We need to exclude deficiency in hemoglobin, vitamins, or an abnormality in other blood tests. MRI brain would rule out conditions such as decreased blood flow and multiple sclerosis (MS). Please get back with any reports you may already have. I hope my reply has helped you. I would be pleased to answer, if you have any follow up queries or if you require any further information. Brief Answer: Thank you for getting back. Detailed Answer: For high homocysteine, you should take HOMIN or HOMOCHECK tablets. For what purpose are you on CIPLAR LA (propranolol)? In neurology, it is used for treating migraine and tremors of hands, but you seem to be on high dose of it. Also, please get back with other reports, whenever possible. Answered by Dr. Sudhir Kumar (1 hour later)
Brief Answer: You are on 3 drugs for high BP Detailed Answer: Thank you for getting back. At a young age, you are on three drugs for BP- Ciplar LA (propranolol), Amtas-e (combination of amlodipine and enalapril). Do you really require three drugs to control your high BP? Taking too many medicines may lower your BP and you would feel weak, lethargic, etc. Also, ask a doctor to check your BP in lying down position and then in standing position. If the BP significantly falls (more than 20 mmHg) after standing, it would then mean you need to reduce your BP medications. Best wishes, Dr Sudhir Kumar MD DM (Neurology)

Brief Answer: Thanks for clarifying. Detailed Answer: In the query details on the right, gender is listed as "female". Still, he is also young. What is the cause of his high BP starting at 22? Generally, it could be related to kidneys or endocrine (hormone-related) at such a young age. Best wishes, Dr Sudhir Kumar MD DM (Neurology)

it started off 2ith panic disorder following which the bp was observed constantly high and that is when earlier inderal 40 was suggested which was later replaced by ciplar la 80.

Answered by Dr. Sudhir Kumar (5 hours later)
Brief Answer: Panic may not be sole cause. Detailed Answer: Thank you for your clarification. Panic causes short-term rise in BP. However, since the BP has remained high for 10 years, requiring 3 BP medications, we should screen for other causes of high BP also. Best wishes, Dr Sudhir Kumar MD DM (Neurology)

Answered by Dr. Sudhir Kumar (7 minutes later)
Brief Answer: Further investigations are needed. Detailed Answer: Please refer to my first reply above. In that brain MRI is pending. For finding out the cause of BP, further investigations would include: 1. Renal artery doppler to rule out renal artery stenosis 2. 24 hour urine VMA to rule out pheochromocytoma. An evaluation by endocrinologist/nephrologist may also be useful. Best wishes, Dr Sudhir Kumar MD DM (Neurology)

Brief Answer: My reply is below. Detailed Answer: Thank you for getting back. First get his MRI brain done, if it shows any problem, then, consult a neurologist. If MRI is normal, you may see a psychiatrist, as he seems to be having symptoms of depression. These two above are tests to find out the underlying cause of high BP, as it is not common for a 22-year old person to start high BP. Best wishes, Dr Sudhir Kumar MD DM (Neurology)

Brief Answer: Report is normal. Detailed Answer: Thank you for uploading the MRI brain and angiogram reports. I am pleased to inform you that the report is within normal limits. This excludes any significant brain disease in him. Psychiatrist consultation would be helpful in making him better. Best wishes, Dr Sudhir Kumar MD DM (Neurology)
