ETD. Throat swab, Sputum examination, Add/ iv antibiotics. CPAP/OSA
Detailed Answer:
Hi,
Thank you for your query.
1. These ear symptoms are related to Eustachian Tube Dysfunction (ETD). Get an Impedance Audiometry done and upload the report.
2. The rest of the symptoms seem to be related to the use of a CPAP and due to the OSA (Obstructive Sleep Apnea). Do a fresh Sleep Study without the CPAP and with the CPAP. Upload the results.
3. Upload the X-rays and CT images.
4. A throat swab and sputum examination x 3 days should be done.
5. Additional or intravenous antibiotics should be started. Anti-allergic, anti-inflammatory, steam inhalation and hypertonic saline washes are recommended.
6. A Videolaryngoscopy (VL'scopy) should be performed.
7. Cancer is not a concern.
8. The ear discharge seems to be ear wax with some mucoid discharge from the perforation.
I hope that I have answered your query. If you have any more questions I will be available to answer them.
Regards.

Hi,
Thank you for writing back.
1. Yes, you can take the Flonase twice a day, but make that one puff (50 micrograms) in each nostril twice a day.
2. Flonase can also be taken as two puffs in each nostril once a day.
3. Taper the use over 3 months. Reduce the frequency to once a month after one month and then once every alternate day for the third month. You can also titrate this dose over a few months up to a year if symptoms reoccur, under the supervision of your doctor.
I hope that I have answered your query. If you have any more questions I will be available to answer them.
