What Causes Fever Associated With Body Pains And Headache?

Question: Hi doctor, On Saturday evening(3 days ago) i started having mild fever along with bad headache and body pain. Since i dint get proper sleep on friday night i thought its due to some exertion and hence took a crocin( paracetamol) tablet and went to sleep. was feeling quite better on sunday morning so avoided any medicine but i felt the same thing in evening so again took a crocin and i was quite ok on monday morning. So went to doctor in afternoon and he gave me Roxid 150 (before food) for 5 days and Lanol ER (after food for 3 days) But in evening again i felt the same. Since this time i dint took crocin mild fever is still there. My fever is on average between 99.8 to 100.2 I dont have nausea and tummy is ok. I m G6PD deficient. Doctor please suggest what next as i have to get well fast. Do i have Malaria ? or dengue or typhoid by my symptom? Pls advise. (Had typhoid and dengue in past) I m 31 ( over weight 90 kg) height is 5.4. Overall i m muscular but have fats on my tummy) Waiting for a quick response. Thanks XXXX

Brief Answer: Investigations necessary to detect the cause... Detailed Answer: Hi, Presence of mild to moderate grade fever associated with body pains and headache favour viral infections more than bacterial infection. And absence of nausea and vomiting makes me feel it is less likely to be dengue. However, I will not be able to confirm my statements without relevant investigations. Viral infections are known to be self limiting. I will suggest you to continue the medications as instructed. Drink plenty of fluids and take good amount of fruits and vegetables (those that are allowed). If you persist to be febrile for the next 2 days, I would order for blood and urine investigations - hemogram, urine routine, widal, smear for MP and dengue serology tests. Hope this helps you. Hi doctor, Thanks for a quick detailed response. Well safer side i conducted my cbc and urine routine and have some doubts. Requesting you to pls help in understanding it. What is mild thrombocytopenia? how it is caused and is it to worry about? it gives me chidslls when i see it relating to low platelets in hiv as searched in google. Also what is Monocytosis? again is it to worry about and how it is caused? Urine report shows occult blood and some faint albumin trace. whats that. pls seecthe attached report and help. regards XXXX

Brief Answer: Features of infection... Detailed Answer: Hi, Apologies for the late reply. Thrombocytopenia as you would have read refers to low platelet count. In your report, I see it marginally lowered. A low neutrophil count and low platelet count are common features of viral infection. Atypical lymphocytes and monocytosis are most likely related to the same infection. Though web relates to HIV, I am fairly convinced its from your current infection and hence you can ignore the findings. Presence of occult blood slightly bothers me. It means when your urine was examined under microscope there was few blood cells seen though the urine was clear. It is most likely the effects of G6PD deficiency and the current infection; it should settle down once you are better. Therefore it may be worthwhile to repeat the test say 2-3 weeks once you are better. I hope you have improved since our last discussion. If you are not feel free to write back to me. Regards

Hi Doctor, I did typhi dot, Widal and infectious mononucleosis and they were negative. Did sonography and it says. Grade III fatty infiltration liver Hepato splenomegaly Also did mantox test but i have doubt. She injected the medicine in my forearm and there was a small bubble. latter suddenly after some time it disappeared. did it get absorbed ? or hit my hand and got burst? Because there was no redness no itch and not even a dot and looked like absolutely same skin i got worried whether the test is properly conducted or not. Pls guide. Also did HAV- IgM Ab to Hepatitis A Virus, serum- by CMIA is NON REACTIVE HEV-IgG Ab to Hepatitis A Virus, serum by CMIA is REACTIVE(11.83) what is this? Currently having a mild fever. Doctor had told me to take Ceftum 500 after Roxid-150 course is completed. Still not met the doctor after these test result. I am worried pls guide. Regards,

Brief Answer: Montoux test is ok... Detailed Answer: Hi, 1. Some test results are significant while few are not. Your doctors have excluded typhoid and mononucleosis with those tests results. 2. The tuberculin units injected into is most likely got absorbed after injected. If the units was injected intradermaly and not subcutaneously, then you need not be worried. Generally if the injection was given intradermaly, one will see a bleb forming immediately; it can get rapidly absorbed. So if you had an initial bled during injection it was most likely injected correctly. If there is a doubt that it wasn't given rightly, you can repeat the tests. 3. HEV IgG reactive indicates you had a past Hepatitis E infection and you are protected against it. 4. Besides this you also have fatty deposits in your liver. Though this is not worrisome, I strongly recommend that you work with a gastroenterologist to make sure it remains stable. 5. Presence of hepatosplenomegaly may be related to G6PD. Please discuss with your treating physician. In all, presence of fever is concerning. It seems the antibiotics have not been effective. You should visit your physician and discuss the course of action. You might also consider visiting a gastroenterologist soon. Hope this helped. Regards
