Question: Hi, I have friend who's been in hospital for 8 days , with lots of test but nothing conclusive , fever rises if not on paracetamol , body sweats , blood shot eyes , very heavy legs , fatigue , he's 43 years old , can you help, he's not been abroad for over a year , he's had a CT scan and brain is clear , he is a smoker and moderate smoker .

Hello,

you haven't mentioned anything about time. When did the symptoms start? If it's just 8 days then it could be something 'simple' like a viral infection. If it's a long time the potential diagnoses are too many.

In case he's having symptoms for more than 8 days various tests may provide help like the following:
- complete blood count to check for anemia and white blood cells count. The counts may provide useful information for a variety of disorders.
- biochemical panel: may help to focus on particular organs. For example increased liver function tests may be a good reason to do tests for the liver, gallbladder and pancreas.
- C-reactive protein, procalcitonin and erythrocyte sedimentation rate (ESR) : these are "inflammatory markers" and may provide additional help regarding the nature of the disease.
- blood and urine cultures: in case a pathogen can be identified.
- basic radiological tests starting with chest X-ray and moving on to abdominal CT scan: they're included in the basic tests for fever of unknown origin.
- certain serological tests like antibodies for HIV and other viruses.
- autoimmunity investigation particularly if ESR is high.
- clinical examination! It may provide very helpful clues to the clinician.
- finally: taking a detailed medical history may reveal the diagnosis in up to 85% of cases!
