Brief Answer:
Get Investigated

Detailed Answer:
Dear friend.
At this blood sugar levels, there are minimal chances of Diabetic Ketoacidosis.
Only matter of concern is your persistent fever.
The flem coming out from stomach might be due to acidity and reflux. For this purpose, you can take a combination of Pantoprazole and Domperidone once a day.
For fever, you should get evaluated. Get following investigations:
1. Chest Xray
2. Hemogram
3. KFT and LFT
4. Urine R/E
5. Widal test
Till the reports are available, you can take Paracetamol as on when required basis.
I would suggest you to start on Pantoprazole and Domperidone combination and to review with the reports.
If you have any further query, please ask me.

Brief Answer:
Detail Follows.

Detailed Answer:
Dear Friend.
So this is a new finding, that is Ascites.
Invariably, this would required evaluation. I would now advice you following:
1. To get an ultrasound guided ascitic tap done and send it for Sugar, Protein, Cell Count, ADA.

2. Get an Echocardiography done to see for status of heart.

Don't take any antibiotics without discussing with your doctor because in case you take an antibiotic which is also an anti tubercular drug, TB would be masked. Since you are a diabetic so there are chances that the abdominal ascites might be Tubercular Ascites.
If you have any further query feel free to ask me.
Stay Healthy.
