Fever with difficulty in swallowing, possibility of Pharyngitis needs to be ruled out.
I would suggest the following
1. Warm saline gargling : To glass of lukewarm water add 2 spoons of salt. Make baby gargle the throat with it thrice a day
2. To a spoon of honey add 2 drops of fresh lemon juice. Make baby sip it thrice a day
3. Antipyretic : Syrup Paracetamol 15mg per kg per dose 4 times a day
4. Urine routine and microscopic examination to rule out urinary tract infection especially in view of Fever and increased frequency of urination
5. Decoction : 1 cup of milk + 2 spoon sugar + 1 / 2 spoon turmeric + 20 Tulsi leaves. Bring to boil. Filter it and feed baby warm ththrice a day
This will reduce her throat pain
6. Syrup Ambrodil plus 2.5 ml thrice a day: This will decongest her throat
I would not recommend antibiotics at present. Do get back to me with urine report.

she has no throat pain now. but fever is on and off. she says that she has stomach pain sometimes. I have given urine culture test and took ultrascan for abdomen.
Urine report will be given only tomorrow she lab has reported that it has to be in ananlysis for 48 hours. Here I have attached the scan report.
Please check and advise.

The ultrasound report suggest possibility of lower urinary tract infection (cystitis ). AS there is persisting fever which is likely to be due to urinary tract infection, I would suggest to empirically start him on antibiotics. Syrup Taxim O 5 mg /kg per dose twice a day will be helpful.
Do get back to me with urine routine and culture report.

Routine microscopy shows pus and red blood cells which confirms the symptoms are due to Urinary tract infection. The urine culture has not revealed any growth of specific bacteria.
So I would suggest you the following
1. Continue Taxim O syrup as advised previously
5 mg per kg per dose twice a day for 7 days
2. Ensure good intake of water : Follow rule of 1.
I cup of water 1 hour before 3 very meals and 1 cup of water 1 hour after every meal . It will ensure sufficient intake of water . Keep following it to ensure good water intake and also will not affect nutrition of baby
3. Hot Seitz bath : To a tub of lukewarm water add 2 spoons of antiseptic solution. Make baby sit in it for 15 minutes. Give bath to baby later on .
This will ensure the external genital are clean and prevent ascending urinary infection. Do it for 3 to 4 days

There is nothing to be worried about liver and spleen. Size. This is normal for child of age less than 5 years old. So be assured and don't be worried.
Urinary tract infection is occasionally seen in girl babies. It is due to short length of urethra. As it is first episode and with normal Ultrasound of kidney and ureter there is nothing to be worried about.
I would suggest the following for preventing
1. Ensure good intake of water . It is very helpful
