Question: Hello
I have been struggling with uncontrolled bp randomly and different times sometimes I check it ... It's perfect others days it is not ... The last 2 years it has become more resistant to meds I take diovan 40 mg daily now I can tolerate more dosage as then my bp gets too low.. The doctors are telling me anxiety related .. Mostly I do have psoriac arthritis I have a heart surgeon tested me for all kinds of stuff EKG fine Slight thickening of heart wall says could be weight and hbp causing that I just found out I have mild sleep apnea using Cpap most nights not 100% compliant I am overweight 70 lbs ( on doctor diet ) Very little success Insulin resistant a1c 5.8-6.2 (I take victosa daily low dose ) Every now and then chest pain But I have so much little stuff they blame that Hiatal hernia Gerd So my question mostly I get cat scanned 1-2 yearly Could this be my kidneys ? Even if cat scan states unremarkable ! I read something about poly cystic kidney disease ? Would that come and go and be missed on a cat scan ? Or I would have it and these cat scans would have seen it ? I'm just so frustrated they just added norvasc calcium channel blocker ugh another pill to relax my Arteries and keep pop up bp from happening One last thing .. Since 2005 I've had a adrenal tumor 1.8cm that has not changed and tested every year non active is what they tell me and not the cause of my up and down hbp Any suggestions or thoughts so appreciated ! Thank you

1. What are the highest and lowest recording of blood pressure. Please throw a light on the trend of blood pressure
2. do you snore at night ? was it evaluated earlier ?
3. do you smoke ? if yes then how much?
4. how much caffeine do you consume ?
5. are you compliant with your medicines?

Yes I am compliant with medication
Quit smoking 6 months ago
Smoked 4-5 cigs daily for 18 years

I do have mild sleep apnea and just started c pap in feb but on that subject not as complient on using it

Highs 170/110
Lows 96/66

Highs are very random not every day

Could this be that adrenal tumor that has never shown active ?

I do have anxiety due to not feeling good off and on

Difficult to control hypertension is most often due to the following reasons

1. Increased salt intake. High salt intake commonly increases blood pressure due to water retention
2. Caffeine intake. Rise in blood pressure is seen in patients who consume large quantities of caffeine.
3. renal artery stenosis. These patients often have very high blood pressure requiring multiple anti hypertensives. Diurnal variation in blood pressure is known
4. Sleep apnea. Which I think is the cause of erratic blood pressure in your case

I don't think you hypertension because of adrenal tumour as these patients are usually very sick, they loose weight and also have cushingoid features.

Here is what you should do

1. Get a 24 hour XXXXXXX blood pressure monitoring ( it's different from holter monitoring). It measures blood pressure every 15 min through out the day, even at night
2. Loose weight
3. Get yourself a sleep study to know how much of sleep apnea is present
4. continue taking your medications and reduce salt intake. Avoid food items rich in salt
