What Causes Forehead Pain Inspite Of Using Amitriptyline?

Question: I have being having pain in the forehead region over the last few months. It can be sometimes a stinging pain on the left or right side and can also feel like a dull ache. When you press on certain areas of the forehead it is tender to touch. Explained to my GP about the forehead and he prescribed Amitriptyline which I have being taken for 2 months. Had a previous CT scan done of my sinuses which showed no areas blocked. What could be causing this?

Brief Answer: Please provide more information as given below Detailed Answer: Hi Mr. XXXXX, Thanks for your query. The common causes of forehead pain are Migraine, Tension headaches,Cluster Headaches, spasm of the forehead muscles; uncommon cause is inflammation of the Temporal arteries (condition called Temporal Arteritis- an autoimmune disease). Your GP has apparently considered Migraine/Tension headache as the possibility and therefore prescribed Amitriptyline. However, the most likely/exact cause can be deduced from additional information, such as, the duration of an episode of headache; does the pain occur at the same time (morning, afternoon, evening or night) every day; its nature (throbbing or dull); associated symptoms like nausea, sensitivity to light; presence/absence of anxiety/mental stress; any particular foods which trigger the pain; and, any other symptoms which have not been mentioned by you. Relief with Amitriptyline would point to migraine or Tension headache as the cause. In the latter condition, increase in the dose of anti-depressant drug, along with addition of mood-elevating drug, such as, Clonazepam or Alprazolam will give near-complete relief. Please provide me additional information as asked for, to enable me to pinpoint the cause of your pain and remedial treatment. The duration of the pain can vary. The pain can last for a couple of hours or until I take some pain medication. The pain is mainly in the afternoon and mainly at evening time. No nausea symptoms and no sensitivity to light. Occasionally eyes can be sore along with the forehead pain. Can get occasional lightheaded feelings more like an imbalance than spinning sensation. Some further information regarding my neck condition below. The neurologist I have seen believes it is caused by tension headaches. A second opinion concluded after a CT scan of C1 & C2 that I have arthritis of the neck joints.

Brief Answer: Symptoms suggest Tension/Cluster headaches Detailed Answer: Well, Mr. XXXX, The additional information narrows down the diagnosis to either Tension Headache or Cluster headaches. I therefore, agree with the Neurologist's opinion, as arthritis of C1 & C2 do not represent the areas where the pain occurs. I would therefore advise that you increase the dose of amitriptyline- in consultation with your treating GP- and add a sedative/mood-elevating drug, such as Clonazepam or Alprazolam. You may also give a trial of Zolmitriptan nasal spray, to be used as soon as the headache begins, and see the response. It will certainly reduce the pain intensity to a significant extent. Take care

Why would I be having sensitive scalp and occasional lightheaded feelings?

Brief Answer: Sensitive scalp is due to pinched nerve Detailed Answer: The posterior 1/3rd of the scalp is supplied by C2 nerve, which arises from C2 part of upper spine. The scalp sensitivity is due to 'pinching' of the said nerve due to arthritis. Occasional Lightheadedness is a common symptom of acute anxiety attacks/mental stress.
