Brief Answer:
Low blood sugar may be the cause.

Detailed Answer:
Hello,
Thank you for asking on HCM!
In a diabetic patient being treated with insuline the first cause of the black out to rule out is hypoglycemia.
So in these moments you have to check the glucose level and if it is below 70 mg/ dl it means that you have to lower the dose of insulin.
If you have ruled out hypoglycemia by checking the glucose level at that moment than a detailed clinical examination, some lab tests and an ECG is needed.
Hope i have been helpful.
Kind regards,
Dr.Mirjeta

Brief Answer:
An aggressive therapy against the high blood pressure is needed.

Detailed Answer:
Hello,
The blood pressure is very high and she needs to be evaluated to exclude a hormonal overproduction as a cause of high blood pressure, a renal disease ect.
If no secondary cause is found, than an aggressive therapy with 4 anti hypertensive drugs is needed.
It is very important to prevent these hypertensive crisis because they are very dangerous.
Wish her good health.
Kind regards,
Dr.Mirjeta
