What Causes Frequent Dizziness, Fatigue, Hair Loss, Short-term Memory Loss And Disorientation?

Question: Age 48
Female
I experienced frequent dizziness in any position for about a month which have now completely disappeared. I feel very flat, unable to laugh or smile naturally and have the occasional suicidal thoughts. I have very recently noticed substantial hair loss as well as short term memory loss and disorientation.
I have very little appetite and do not drink enough fluids on a daily basis. My brain feels very foggy about 80% of the time. Unable to make decisions and the occasional slur in my speech. On the upside, I have an extremely healthy sex life and hardly ever get sick or experienced headaches. I am a smoker
I have a history of brain aneurysm on my fathers side, which a 1st cousin on my fathers side has had and survived. My last extensive health test was over a year ago and my blood texts showed nothing abnormal. Vitamin D good and iron fine etc.
I still experience regular menstrual cycles
Should I be concerned? The memory loss is quite debilitating and I feel very anxious about it as I tend to forget things and cannot recall some things (short term) at all.
Should I see a doctor?
