What Causes Frequent Episodes Of Passing Out And Joints Pain In Children?

Question: Ok, here ya go! I have a 9 yr old neice who has been passing out resulting in 4 concussions in 4 months. They found a unexplained "spot" but haven't been seen by a specialist!! Meanwhile she has also had joint pain, random fevers and acid reflux! Yesterday, she woke up at 4 am with chest pain....she said, like someone sitting on her chest. Looking at her.....she has a VERY ODD, SYMETRICAL PATTERN, "+"S SHAPES, RASH
going across her chest from arm to arm approx. 3 inch in height. Can you connect the dots? No one else can.

Questions -

1. Did she lose consciousness and fall down or after falling she lost consciousness?

2. What do you mean by finding an unexplained spot please elaborate?

3. Can you upload the images of the rash which you are quoting?

4. Is there any family history of seizure disorder?

5. Does a rash appear over the malar surface on the face?

6. Is there any history of connective tissue disorder in the family like SLE?

1. she just loses consciousness (once while riding her bike, once on staircase, other two times where just while playing)

2. they did a brain scan 2 months ago (socialized med.) are still waiting to see specialist for a reading. UGH!

3. ? will try to upload photo ? not sure how to

4. No family history of seizure disorder

5. No rash on face

6. No history of connective tissue disorder

.....note: she was admitted again today, for observation and more test.
the rash is slowly continuing to spread .... only on chest, abdomen
front of thighs.

Looks like vasculitis and vascuitis induced central nervous manifestation

Detailed Answer:
Hi... I have just seen the images of the rash - I put forth these possibilities -

1. Vasculitis as an auto immune phenomenon and autoimmune vasculitis induced infarction in the brain which is the irritative focus in the central nervous system which is making her loose consciousness. If the brain scan is also showing a spot, then, all this this becomes more relevant keeping in mind her fever and joint pains.

2. The symmetrical plus shaped rash and the symmetrical dots lower down in the abdomen are looking like self inflicted or could a scratch induced marks if there is itching or it could also be a child abuse. Please keep in mind that this is only a possibility and please do not get offended. As s doctor in view of the child's well being I need to consider all the possibilities and do justice.

3. Contact dermatitis - but this is least likely possibility as it would not explain the joint pains and fever.
