Question : I corresponded with 2 of your doctor's in the past week. I began having mild bladder symptoms about 10 days ago, mild discomfort and some frequency. I started cephalexin 500mg twice a day for 7 days on Tuesday because urine showed 75 wbc and 100 protein, no nitrites. . So been in them 4 1/2 days. Had urine retested yesterday. It was totally clear of everything. However, the slight discomfort and frequency are still there. I am very worried and concentrating on the area continually. I know I get frequency when I'm stressed. Could it just be that. The doctor thought the symptoms should be gone but, when urine was completely clear, she was unconcerned. Also the pH has been 5. My friend, a PA suggested maybe I back off on cranberry, vit C etc... As I was trying to make it acidic. She felt that may be irritating

Your concern about the bladder symptoms is not something to worry about. As a
Urologist, i am assuring you don't need to worry.You're stressed.try to relax.Do exercises and try meditation.Maintain 8-10 glasses liquids daily.Don't try to make your urine acidic. That is done by the kidneys anyway.There's no need to take cranberry juice or vit C. Some people find relief with lime juice.If your blood tests like s.creatinine and blood sugar are normal,you have nothing to worry at all.There's no need to keep checking your urine either.

I appreciate your detailed answer. However, I haven't had blood work done in 2 or 3 years. I asked my Dr about it but as all was normal the last time, he felt 3 years was enough. He didn't think this incident warranted anything further. Also, as the protein went right down, doesn't that mean the kidneys should be okay? Sugar and a1c have always been good as well, though. Due to have it checked soon but should I be worried about this? Especially as the symptoms came with the indications of infection in urine and urine cleared with antibiotics? The littlest suggestion gets my mind spinning, as you can see. Also, I was wondering... I haven't gotten the results of the culture back. There were no nitrites but the high wbc and protein, as I said. Now it appears that's normalized. But, if the culture shows no infection, what should I think? Would that even make sense?
Since you're a urologist, there's another concern I've had. My last blood work showed creatinine of 1.1 then 1.0 later. The egr or egr, whatever, was a bit low on one and just inside the range in the other. My doctor said not to worry as its just a calculation. I obsessed over that at the time. And I always wondered, if they calculate it based on age and the creatinine remains steady, that calculation will keep going down. I'm afraid to even have it tested because now I'm even older! I asked a doctor friend who said he wished they wouldn't even do that as its merely a calculation and he'd prefer just to look at the actual kidney markers. That was the first time a lab ever calculated that. Must be relatively new and very unsettling... As I looked at it myself.

Hi again XXXXXXX if your urine culture shows an infection, it should be treated with a course of antibiotic.As s. creatinine is sensitive test for kidney function, you don't need to worry about that either.You're as young as your mind is.63 is certainly not in the "old" category.so relax.

Regarding the first question, I was treated with cephalexin 500 mg twice daily based on 75 wbc and 100 protein but no nitrites. One Dr said irritation, not infection, the second said infection and sent for culture. That's why I asked what if culture shows no infection? What does that mean? However antibiotics did rid the urine of wbc and protein but not all the symptoms.
Referring to second question, do you agree with my doctor that my blood work should be still okay? He didn't see a need to get tests again now. Or should I worry that these symptoms signify a change? Neg or trace protein is a good sign isn't it?
And, as long as creatinine is 1.2 or less (mine was 1.0 Nov 2012), it doesn't matter if it doesn't come down with age which lowers egr? At 63, the formulas want you to have really low creatinine to get into the best ranges.

Only if the culture is positive, you'll need antibiotics. culture negative means no more treatment required.
Secondly, blood tests for health check-up are advised annually.As it's 2-3 years, since you had a blood test,i would surely recommend a repeat test of s.creatinine and sugar.

I realize I'm not making myself clear. The culture was done on the first urine with wbc and protein, not on the second that was clear. So if it is neg but the antibiotics cleared up the wbc and protein then is it still an infection? Haven't gotten results.
And, if urine is clear, should I reasonably expect blood to be okay? Is your suggestion routine or is there cause for concern based on our discussion?
Once this discussion is closed, how would I contact you? They just give random doctors.
Still unclear why the internet places such importance on egr when I don't see how it can be right when age factored in.

Once the urine report does not show wbc's or protein, you can assume to be free of urinary infection.The blood and urine tests are not related in any way. Forget the egr.It's a age old calculation, that's not in routine use. s.creatinine is very sensitive indicator of kidney function.As your report is 1.1, it's perfectly normal.
Annual blood HbA1C check is a routine health check.Better to get it done and clear your doubt about diabetes.

Thankyou. I'm not debating the need for routine screening... Just asking if I should rightly be relieved that my kidneys are most likely fine based in the recent urine and not fear that the blood would be vastly different from what it was before. In other words, I have know doubt that you have gleaned from our discussion that I obsess over these things and I don't want to be expecting a problem because of these symptoms. I'm still urinating alot and feeling the tingling irritation... Though I am completely focused on it. And it started when wbc and protein was up. Before that urine frequency was normal other than occasional increase from anxiety or lots of water. Antibiotics cleared that (along with some back discomfort, that the Dr checked and didn't think was kidney, and very slight temp increase) but still this other stuff which I guess you agree can be anxiety.

Maintain 8-10 glasses of liquids daily. By drinking more liquids, you'll have to pass urine more frequently, as your kidneys are functioning normally.

Thankyou.... I get how anxiety can cause frequency, but also that tingling irritation that I can often feel in between? ... But, again, I am focused on it. Could thatalso be dryness from post menopause? I truely appreciate your patience and will give you an excellent review.
And if I still have symptoms but urine is now clear, how will I know if I get another infection? ... Perplexed.
Well now the internet has me all worries about bladder cancer. "If you have a bladder infection and still have symptoms after antibiotics, get screened for bladder cancer." "Frequent bladder infections can be symptom of bladder cancer"
