Question: 1. Doctor, I have to go to the toilet frequently. 12 times in a day – Every 1 hour or 2 hour almost. Especially after meals. I have to run to the toilet immediately after meals. Lunch Breakfast Snacks or Dinner. Also I am unable to control bowel movement. I need to run & go. Else I spoil my underwear. This causes problems when I am outside. As a result of which I am unable to Also I am not able to differentiate between a gas discharge & stool discharge. There might be an unexpected stool discharge along with gas or while I am urinating. Also it foul smells. Smells like fermented curd discharge. Bowels are also foul smelling. After using the toilet also the smell remains. Anus gives a strange smell, due to which it becomes very embarrassing in the office. I also experience pain on the left side of my lower abdomen & I am unable to sleep on my left side. I have to either sleep straight or on my right side. I have been diagnosed with Irritable Bowel Syndrome. Is there any remedy / permanent cure for this ? Please suggest remedy / medicines. 2. I am also suffering from acidity & indigestion. I feel like a burning sensation in my throat while eating / swallowing food, even fruits or banana or even water. I also feel like a burning sensation in my throat for most part of the day, for many hours after eating. I have lost 15 kgs of weight in the last month. & reduced to a skeleton of bones. My pants hang loose for me. Please help me gain weight / restore to normal. 3. I am suffering from bleeding from my anus in & after passing of tools. I have been diagnosed with Proctitis & Piles. Is there any remedy / permanent cure for this ? Please suggest remedy / medicines.

1. Was stool test done? - YES 2. Was colonoscopy test done? - NO. IT IS VERY PAINFUL. THEREFORE ONLY CAMERA TEST WAS DONE. It was just a 6 inches metal device which was inserted in my anus to see the rectum. 3. Are you a diabetic? - NO 4. Are you on any medications? - NO / YES. I WAS NOT ON ANY MEDICATIONS AT THE TIME I STARTED SUFFERING FROM THE DISEASE. HOWEVER I TOOK ONLY ALLOPATHIC MEDICINES LIKE Enzaar Forte, Flaggyl 400, Immodium & Daflon 500 were suggested for 16 days FOR CURING PROCTITIS / PILES TREATMENT. THEREAFTER I HAVE STOPPED ALL ALLOPATHIC MEDICINES & I HAVE BEEN TAKING ONLY AYURVEDIC MEDICINES FOR PILES & DIGESTION. Brief Answer: Answer given below Detailed Answer: Hi, Since you have bleeding per rectum I would advice you to get a colonoscopy to find the cause. Though piles can cause bleeding a colonoscopy has to be done to rule out other cause like IBD.. You can get it done under sedation and you will not feel any discomfort. So can you not suggest any medication without colonoscopy done ? But before getting colonoscopy done, I would like to know that : Is there any permanent cure / medicines for IBD, if IBD is ruled out ? If not, there is no permanent cure / medicines for IBD, then there is no point in going through the entire pain & getting the expensive colonoscopy done. How would taking Nexpro-L once a day before breakfast for 8 weeks help ? Please advise. Regards

What are the results of my stool test ? What do the reports say ? 2. Am I diabetic ? 3. I did make a mention about my medications. Are they causing the disease ?

Brief Answer: Answer given below Detailed Answer: Hi, Sorry for the delay in replying. In this era of internet people try to get more information than they need to know and eventually get confused. So sometimes a short and sweet reply is more appropriate than a long exhaustive answer which may confuse the customer. Since you expect lot of details, I am giving the all the information you want. In your first post you asked three questions. Though the first part suggests more of irritable bowel syndrome, there are three points against it - 1. Blood in stool 2. Weight loss 3. Fecal incontinence ( soiling of your undergarments). Blood could be due to piles. But takinga ll the other information together, IBD has to be ruled out. You have mentioned you have proctitis. Proctitis could be a part of IBD. IBD means inflammatory bowel disease. It is not the same as IBS which is irritable bowel syndrome. For diagnosing IBD you need to undergo colonoscopy. You asked whether there is a permanent cure for IBD and also said if there is no permanent cure there is no need to get a colonoscopy done. It does not work like that. For example diabetes does not have a permanent cure. But still you need to treat it to prevent complications. Similarly acid reflux does not have any permanent solution and can recur many times. But still you need to treat it as it affects the quality of your life. So you need to treat IBD also even if there is no permanent cure. Because it can affect quality if life and can lead to complications. It can assciated with other symptoms involving the joints, eyes and skin and treatment would control this. Also there is a long term risk of malignancy. Now if you ask me why you need colonoscopy - Bleeding from any site is a red sign. Since you have bleeding per rectum you need to get colonoscopy to rule out IBD as said though you have piles also. Colonoscopy will help to confirm IBD if it is the cause for your symptoms. The doctor would do biopsy to confirm the finding. Moreover there are two different types of IBD- Crohn's disease and ulcerative colitis. The treatment is different for these two. If you suffer from IBD colonoscopy will help to know which of these two you have. I told you to take Nexpro-L for acid reflux and you asked how it is going to help. Normally acid is secreted in the stomach for digestion of food. But this has to go in the downward direction. If it goes upward in the opposite direction, it will enter the food pipe and cause symptoms like heartburn which you have. This is called the acid reflux. In the stomach, there are microscopic structures called proton pumps which are responsible for acid secretion. nexpro-L contains medication called esomeprazole which blocks these proton pumps. This leads to decrease in the acid secretion. So less or no acid gets refluxed in to the food pipe and you get relief from acid reflux/heartburn. You said I asked so many questions . I asked these questions to analyse your symptoms. I asked about the stool test to know whether you have any infection as infection can cause some of the symptoms you have mentioned. Your stool report says you leak blood in stool. Other than that everything else in stool test do not reveal anything significant. I asked whether colonoscopy was done just to make sure whether IBD was ruled out and I have discussed about it detail above. Then I asked about diabetes because diabetes and some of its medications cause some of the symptoms you have. You yourself said you are not a diabetic. As per the report you do not have diabetes. I asked about medications to know whether you take the right medicine. So I suggested Nexpro-L. None of medications you take cause any of the symptoms you have. Hello, Please do not judge the sufficiency of information required by the customer. Leave its adequacy to be judged by the customer himself. The need for information is not due to internet age, but due to conscious & intelligent customers. The doctor whom I had consulted in the hospital wanted to operate me. He wanted to do surgery on me without advising me of the disease I am suffering from, just because I have Bajaj Allianz Health Insurance. At that time I did not have any symptoms of IBS or IBD. He did not inform me of the degree of piles & just wanted me to get hospitalised. Admitted in First Class because it is a major surgery (because I have Bajaj Allianz Medical Insurance). The hospital has standard laminated rate cards for Individuals, Corporates & Mediclaim patients with different rates for different class of patients. The doctor asked me upfront in front of everyone, if I had mediclaim & changed his rate (fees) accordingly. So it is better that the doctor maintain transparency with the patient as it is his life & question of his body that he would seek to operate for money, rather than the money. What is the remedy for fecal incontinence. It is a big fiasco in office or when I am outside. Does colonoscopy have any adverse effects ? Like increase in incontinence / colitis etc ? Regards

You asked how Nexpro-L is going to help relieve the symptoms of acid reflux. I do not know how it matters to you. You said if there is no permanent cure there is no need for colonoscopy and for that I gave you some examples why your thinking is wrong. Here in healthcare magic everything is transparent only. Now coming to your questions - There are many causes for fecal incontinence - IBD, IBS, Fecal impaction, damage to anal sphincters and neurological cause. The treatment depends of the cause. Fecal incontinence is a symptom and not disease and the treatment is diverse and depends on the underlying condition. Since you said you have proctitis, it could be the probable cause. But again proctitis has many causes like IBD, infection etc. Your stool test does not reveal any infection. Colonsocopy does not have any adverse effect other than the discomfort and I have suggested you to get it done under sedation so that there is no discomfort. It will not worsen any symptoms. I would advise you to consult a qualified gastroenterologist ( I presume you are from Mumbai).
