Brief Answer:
Need treatment adjustment

Detailed Answer:
Dear Mrs,

You need treatment adjustment for the following conditions despite your treatment;
1. you have resting heart rate of 85-110 which is high for congestive heart failure
2.you have blood pressure drops and maybe orthostatic hypotension
3. you still have anxiety and pain
My advice will be to see your doctor and discuss together possibilities of treatment changes

Have a good health
