Brief Answer:
Need few routine investigations

Detailed Answer:
dear,
Welcome to HCM
As I see your chief complaints are "fuzzy head", forget fullness, fatigue
I note dat you have a past history of Crohn's disease and chronic kidney condition (as mentioned in medical history), your age is 72 years.
Crohn's Disease and Kidney conditions both predispose to nutrient and electrolyte imbalance, that may cause deficiency of important nutrients and minerals. Tiredness, fuzzy head and forgetfulness are all explain able with this. Therefore, I would suggest to get a Vit B12, serum iron, serum folate, electrolyte levels checked. It can accordingly be corrected.
Secondly, Crohn's disease being a chronic inflammatory condition is often associated with persistent tiredness and anemia.
Thirdly, I would also want to consider age related or independent neurological condition that could cause forgetfulness and associated symptoms. In regards of which I would need to if there as asociated complaints of difficulty in walking, forgetting roads, household tasks, shaking of hands, etc. An MRI brain or CT scan of head can yield us important information in this regard.
Finally, if the condition persists I strongly advice to consult a local neurologist as clinical examination provide us with clear diagnosis.

I hope to have satisfied your queries.
kindly revert back if more information and if any query.
Regards,
Dr Pankaj
