default
Posted on Mon, 25 Aug 2014
Question: I had an EUS with biopsy.
There was a contained esophageal tear 3mm at GE junction.
After this I hear crackling after I swallow when lying down only.
It wakes me up several times at night.
Barium swallow normal.
The crackling sounds like air going through fluid.
I have tried many medications for sleep and drying up fluids.
Please help.

Brief Answer:
gastro - esophageal reflux

Detailed Answer:
Dear madam,

Thanks for your query.

By your description it looks like you are having gastro - esophageal reflux that means reverse movement of contents in stomach like food, water and air. This is due to tear in oesophagus causing loss of tone at gastro - esophageal junction which prevents reverse movements of stomach contents. If I were your treating doctor i would have prescribed proton pump inhibitors with prokinetcs drugs 1-0-0 for 02 weeks. Take these drugs half an hour before breakfast. Take syrup sucralfate 3 tea spoons half an hour before lunch and dinner for 02 weeks. If symptoms persists then contact gastroenterologist.

Hope I have answered your query. Thank you.

Yes, I was prescribed this. I forgot to mention that I was on carafate for 2 months.

Now having another EGD and PH study.

This unusual sound is really scary and sounds like snap crackle pop and it comes from my throat.

NO ONE seems to have seen this before.

Another doctor thought it was a diverticulum, but it did not show up on the barium swallow.

Should I show the video that has the sound on it to the doctor to show them HOW UNUSUAL it is?

Thank you.

Brief Answer:
Newer generation prokinetics and anxiolytics

Detailed Answer:
Dear madam, there are newer generation prokinetcs available with better efficacy. Consult gastroenterologist. Ya you can show the recorded sound to your gastroenterologist... Don't panic to present problem. Ideally it will take 6-8 weeks for relief of symptoms. Take some anxiolytics just to ease your thoughts. Hope I have answered your query. Thank you

anxiolytics

Detailed Answer:
Dear madam, anxiolytics will calm you, relieves your apprehension about disease... Don't worry... You will be fine... Just continue treatment for 6 weeks... ( proton pump inhibitors , prokinetics, sucralfate ). As I told before avoid spicy food, have food at regular intervals, avoid over eating and beverages. Hope I answered your query. Thank you
