Question: I can't sleep Ive got a really heavy head, tight chest/neck, ringing in my ears, my arms feel weak, I cant breath very well and I have multiple sclerosis. please can you advice me it is scaring me

Hello Mr XXXXXXX

I was diagnosed with Relapsing Remittant M.S in late 2011. I take gabapentin for the pain which may occur and Fingolimod for the M.S itself, I have recently seen my specialist where we discussed how effective the treatment has been so far with no new relapses (Been using Fingolimod for about 2 years previously was on Copaxone).

I was really struggling to sleep and felt really spaced out and the symptoms suddenly came on and increased in itensity, I also normally take about 1 hour roughly to fall asleep.

Thnaks for writing back,

I apologize for the reply being delayed, had to get in contact with a neurologist to discuss the case so that I could come up with a better response , more helpful for you.

My opinion:

After reading your history and description of symptoms there the following possibilities:

1) Anxiety:

It might be your anxiety about your disease exagerating the symptoms.

2) Acute exacerbation of MS:

These symptoms are also quite characteristic of MS relapse, as I can not examine you or run labs on you so I will suggest you to visit a facility even an ER would do, if you can't get immediate appointment, if their neurologist on call thinks its MS, just one steroid shot will relieve your symptoms big time.

3) Drug adverse effects:

This can be adverse effect of drugs you are using , although you are using them for quite a long time so this possibility seems remote.


END NOTE:

Here I want to assure you that this does seem to be a controllable issue if it's anxiety an anxiolytic pill will do, if this is relapse a steroid shot will do , it is least likely the drug, even if it is, it can be corrected.

But for that i will suggest you to visit your doctor. To make sure there is no serious issue.

The neurologist I talked to said it's common to get relapses in relapsing remitting type of MS, we can control it easily.

You can also post your query in our "Ask a specialist " panel and chose "neurologist" as speciality , you can directly discuss with neurologist if you can not get in contact immediately with your neurologist, which indeed is preferred.

Thank you for contacting a neurologist for me and for your detailed feedback.

I feel much better now, I was a bit confused this morning with the painkiller dosage last night, I think the problems I dealt with last night were due to a combination of taking a double dose by accident and as you mentioned anxiety. I have had relapses before but not in that manner it could be a M.S blip too.

that is why i mentioned drug side effects as one possible cause though it wss unlikely because you have been taking it for quite some time.

But as you took overdose so drug toxicity became the prime suspect.

Relapses for different people present differently so I mentioned it as a concern.
