Well, frankly you have named symptoms that are broad and widely spread across the body which makes it difficult to arrive at a conclusive diagnosis. However there are few possibilities that I think should be considered.
1. Chronic headache such as migraine presents with wide range of associated complaints. Symptoms such as blurred vision, ear ringing, feeling of detachment, brain fog, numbness, balance issues, etc., etc. Since these are episodic symptoms, you very well can just be suffering from chronic headache and these are its associated complaints. The vision changes are also related to migraine attacks.
2. It may be linked to drug side effects. You are on a list of medicines that include anti hypertensive, heart rate lowering drug, steroid, bronchodilator as well as for prostate hypertrophy. Though all the drugs play an important role and cannot be missed, side effects and some interactions are a possibility.
3. Anxiety and panic related issues also need to be mentioned since you do report XXXXXXX panic attacks on some occasions.
