Question: Hello Doctor, I am a 57 y/o male, law professor. I am a retired army colonel and sustained injuries resulting in numerous surgeries. I had a major surgery this last April to fuse my upper fibula to the tibia/lower knee with screws and bone grafts. Tough recovery. I have been taking a very low dose ( low dose based on my desires) of hydrocodone/Tylenol 5/325 T.I.D for six months. I also take klonipln (3mg daily - been taking same dose for 12 years for "panic disorder"). Additionally, I have been on 200mg 1xdaily of Welbutrin for 7 years (less than common therapeutic dose I know). Yesterday my doctor added 50mg of Tramadol T.I.D.. I discussed my concerns about seizures and agonist opiate withdrawal, she ( doctor ) laughed at me saying" you do not have a seizure disorder and you are taking a very low dosage of hydrocodone so do not worry about either concern! I took Tramadol 8 years ago for a short time ( two months) ( same doctor) only difference then was I was "not" also taking low dose hydrocodone. ( I "was" taking same dosages of klonipln and Welbutrin) Only problem back then ( when taking Tramadol) was occasional headaches and (seriously) my butt checks, and only my butt cheeks inched like hell!! Need a second opinion!! Thank you

Brief Answer:
Tramadol can be an add on indication for chronic moderate pain

Detailed Answer:
Hello, Sir.

I can understand your concern.

The risk of seizures is extremely low Tramadol.

Its incidence is <1%.

Dosages not exceeding 400mg/day is safe.

A dosage of 50 mg TID is relatively safe for 1-2 months.

Occasional headaches are common with tramadol and resolve spontaneously.

Post your further queries if any.
Thank you.

Can you address the incidence of tramadol causing opiate "withdrawal like" symptoms when also taking "particularly" low doses of hydrocondone? The pharmacist stated this possibility to m? ( I would not know the symptoms as I have never experienced withdrawal)
Thanks
