Brief Answer:
Ear drum injury to be ruled out

Detailed Answer:
Hello dear,welcome to HealthcareMagic forum.

Decreased hearing following fall on ear in water may indicates an ear drum injury especially if there was blood stained discharge from that ear(this may not be evident as he was in water for sometime after injury). This needs immediate evaluation by an ENT specialist. If there is some tympanic membrane injury, this may need surgical repair depending on the size and type of injury.Fresh wounds are easy to repair, and heals fast.So avoid any delay.Minor, clean injuries needs only supportive treatment with antibiotics and analgesics.

Any unilateral disturbance in functioning of auditory apparatus can cause some disturbance in equilibrium also. This need not be worried. It will be corrected its with management of the primary insult.
Kindly see an ENT specialist as early as possible.
Hope I have answered your query.
