Question: Hi. The past 1-2 years i have been having some problems with my heart rythm. Most of it is probably just PVC and i understand they are not dangerous. But i have also been having sudden very fast heart beats. I can be relaxing with 50-60 in pulse, and suddenly it jumps to 100+ for 2-10 seconds. I have finally been able to record this and would like an oppinion on it.

Other symptoms: Sudden dizziness, pain in hands and fingers, and alot of stomach problems like bloating. I read about something called Romheld Syndrom, where the stomac can affect the vagal nerve and can lead to heart symptoms.

I also notice that when i go from sitting to standing my heart rate immediately jumps from 50-60 to around 120. Any small activity creates a very big increase in heart rate. My relaxing heart rate can also get very low, low 40s somtimes.

I have uploaded a smaller pdf showing the time when i got this sudden spike in heart rate. It happend as i was going to bed, and as soon as i laid down on my right side.

I carefully reviewed your ECG reports and would explain that the only finding in these ECG is some ectopic cardiac beats.

There is nothing to worry about! In a structurally normal heart (resulted from cardiac echo), some ectopic cardiac beats are non significant and could be related to a metabolic disorder (chronic anemia, thyroid dysfunction or electrolyte imbalance) or just pure anxiety.

I would recommend performing some blood lab tests:

-     a complete blood count for possible chronic anemia
-     thyroid hormone level for possible thyroid dysfunction
-     blood electrolytes for possible electrolyte imbalance.

Regarding Romheld Syndrome, it could explain ectopic heart beats while abdominal distension or esophageal reflux. Nevertheless, if you maintain under control your gastric symptoms, there is nothing to worry about. Some tips would be helpful:

-     avoid spicy food and alcohol
-     avoid smoking
-     take antiacids or PPIs (omeprazole, lansoprazole,etc.).

This syndrome associates digestive disorders (like gastro-esophageal reflux, abdominal distension) to cardiac arrhythmia and changes in blood pressure. Your ECG findings are not significant for possible cardiac arrhythmia.

From the other hand a fibrogastroscopy would help exclude possible disorders of the digestive tract.

Regarding the increase in your heart rate while standing up from sitting position, I would explain that these symptoms could indicate possible postural tachycardia syndrome (POTS). You should know that an increase more than 30bpm during standing up from sitting position, without any decrease in blood pressure, would be indicative of POTS.

A head up tilt test is necessary to rule in/out this possible disorder.

You should discuss with your doctor on the above issues.
