Question : I've been sick for about 2 months and my dr is not coming up with anything particular. I started out in XXXXXXX 2015 morning and had an overwhelming heat sensation in my heart that rushed to my extremities then I felt weird like I couldn't see or felt like I was dying. I pulled over in my car and called 911. The ER de said it was a mixture of cough suppressant and a 5 hour energy that was hours away from one another. I've tsken both before and was fine. My blood pressure went up and down for about 3 hours and every time I had an episode that day I felt heat coming from my heart to my extremities. I felt like I was being boiled alive. They said I had to let it run it's course. After about 8 hours in the er I felt better and they released me. Well since then I've have at least 10 or more episodes. My mouth, lips, face,- and eyes have a menthol feeling. I am currently seeing a dr who is checking me for things that I have no clue about. He don't an mri this past week. I'm tired and weak. I'll start to feel better then it comes back. I was thought to have had a seizure last week(history) from years ago because of the particular episode. I was sitting on my porch and felt my right ear go numb and my body once again had some heat. I began shaking and my palms of my hands were so sweaty. I can't sleep no more than a few hours at a time. I'm just a mess. If is panic disorders then I'l get meds for it but at this time I'm on keppra 1000 mg a day. I'm having such a hard way to go and have no clue what is going on with me.But the dr did say I had a borderline positive test result for an autoimmune disease but he didnt say what kind. Thanks

I read your question carefully and understand your concern.

Your symptoms seem to be related to an epileptic seizure or a panic disorder.

I would like to know more about your previous seizures and the time of your beginning. Referring to your past medical history, positive for seizures, these episodes are probably partial seizures.

There are different types of seizures. The symptoms you describe are very similar to an epigastric aura, typical for temporal lobe epilepsy (which is a partial epilepsy).

The problems with your taste (menthol feeling) in the mouth are too related to the deep temporal lobe, where lay the brain centers responsible for the taste.

It is true that energy drinks can trigger a seizure, but also a panic attack.
But an autoimmune disorder, causing an inflammatory reaction of the body, could also exacerbate seizures.

A brain MRI and an EEG would be very important for the differential diagnosis between a seizure and a panic attack and to identify a possible intracranial cause (a lesion in the deep temporal lobe).

Other tests like: a routine blood test, inflammation tests (fibrinogen, PCR, sedimentation), kidney and liver function tests, thyroid hormone levels, blood electrolytes, are needed to establish the possible cause of these episodes (or seizure exacerbation).

Other specific tests for autoimmune disorders (ANA, XXXXXXX ENA, RF, etc) should be performed after consulting with a rheumatologist, to investigate for any possible connective tissue disorder and the treatment options.


If after the EEG, it is concluded for seizures, I recommend discussing with your attending physician the possibility of a dose change to Keppra therapy (this is a small dose) or to add another antiepileptic drug (like carbamazepine, oxcarbazepine,etc).

I've not received my test results back from mri. I did get a good eeg and nothing shown in the eeg. But i wasn't having an episode and was having a good day. Years ago I was told it was anxiety and not really seizures. So from that point on I just assumed anxiety disorders. I've had the tingling and numbness in my lips today and left hand today like I had last week. But not as bad. Just tired of feeling bad. I'm an outgoing person and not a sedentary individual. The menthol had been coming and going all day. More at night for 2 days. My previous blood work had good results as far as thyroid and cbc. Other than the borderline positive autoimmune disease. My endocrinologist wouldn't let us know what he's looking for exactly. He's checking for pheochromocytoma as well. He said its rare but he wanted to check for that as well. 24 hour urine test i have completed as well. Is there anything I may need to suggest to my dr. Just want to figure this out. I've missed work for 3 weeks now and can't drive for 90 days. This is horrible. Thanks so much

Temporal lobe epilepsy with epigastric aura not always show EEG changes, because the epileptic focus lays deep in the brain and the distance between the electric discharges and the EEG electrodes may be not short enough to detect these discharges in the EEG.

Your past medical history for seizures is a little concerning.

It is necessary to perform a continuous 24 hour EEG, to exclude the seizure occurrence.

From the other hand anxiety could mimic this clinical scenario as well.

Pheochromocytoma is another disorder which can simulate panic attacks, but is also associated with other symptoms like episodic uncontrolled high blood pressure, face flushing etc.

If the urine tests result normal, this disorder can be excluded.

If the MRI and 24 hour EEG result normal, I recommend consulting with the psychiatrist to consider the possibility of starting an antidepressant (SSRI), which are very effective against anxiety.

This could serve also as a therapeutical test, because it is known that antidepressants can decrease anxiety and the frequency of panic attacks, but can exacerbate epileptic seizures.

If such complication occurs, it may be because of hidden epileptic fosus in the brain , undetectable by the EEG or MRI and a higher dose of levetiracetam may be necessary, besides stopping the antidepressant.

Thanks so much. I did have anxiety meds prescribed back in July and it caused me more issues. It was like I was getting adrenaline rushes. I would be sleeping snd wake up with the heat in my heart again and going to my extremities. Then my blood pressure would go from my normal Bp:example 92/60 to 183/104 heart rate 90s-108 and that's resting. So that was after taking the meds for about a week. It caused the flushing and sysmtoms worse. I would then get sweaty hands and feet. Then shake all over. I felt so horrible. After I stopped the celexa and buspirone the episodes became less but still there. Thanks.

The episodes of high BP and face flushing may be related to the higher serotonin levels in your blood caused by celexa intake.
It seems to have been serotoninergic syndrome.

Taking into consideration this information, I recommend consulting with your attending physician on the possibility of adding another anti epileptic drug like carbamazepine or oxcarbazepine to your therapy, or increasing the dose of levetiracetam, despite your normal EEG.

Hello I have an increase in the menthol feeling again on my lips and tongue. I have a heaviness in my lungs and a cough. I have a weird taste going on today. I haven't really been sleeping for the past 3 days. Literally im waking up after about 1-3 hours of sleep. I called my dr office and they still haven't received the mri results. I just feel so bad and pray this goes away soon. I'm actually going to a mental health counseling on Friday. I'm going to reach out in this direction as well. It all my be anxiety. But one way or another I'm going to get this figured out. I was told by a friend today they had a b12 vitamin deficiency and had some of the same symptoms.

It is true that vitamin B12 deficiency can be associated with your changed perception in the tongue (also called dysgeusia) and can be the cause of seizures exacerbation.

But it would also be associated with chronic megaloblastic anemia and other symptoms like numbness in the limbs, etc..

I advise performing vitamin B12 plasma levels to exclude this possibility.

Regarding anxiety, if all the tests result normal, it remains the most probable diagnosis.
